I, __________________________, attest that as an applicant for admission to the TEEX Unexploded Ordnance Technician
Level - I Course, I have met the following requirements and answered all questions truthfully:
1. I am at least 18 years of age.
2. I have been through a comprehensive national background investigation.
3. I have not been convicted of any violent crime or felony within the last ten years and I have disclosed all felony
convictions outside of that time period.
4. I am not under indictment for any criminal offense.
5. I am not prohibited by law from possessing or handling explosives.
6. I have been examined by a licensed physician and declared in writing within the past 45 days to be both physically sound
and free from any defect which may adversely affect the performance of duty as an explosive ordnance technician, and
show no trace of drug dependency or illegal drug use after a physical examination, blood test or other medical test.

9. Have you ever been convicted in any court of a felony, or any other crime, for which the judge could have imprisoned

10. Are you under indictment or information in any court for a felony, or any crime, for which the judge could have
