You are hereby given notice that the undersigned is terminating your tenancy of the premises

This termination notice is as a result of inter alia, the reason(s)/violation (s_ by you as mentioned

Pursuant to Section 34, Chapter 562A, Title 14 of 2011 of Iowa Code of the lease deed, I
terminate this lease and I request you to refund the rental advance subject to adjustment of
arrears of rent. In case of any delay in making the refund of the rental advance, you shall be

You are required to vacate and surrender possession of the premises to the undersigned on or
