1     ORGANISATIONAL BACKGROUND

The Leeds Teaching Hospitals NHS Trust is amongst the biggest in the UK
and includes two of the largest teaching hospitals in Europe, St James’s
University Hospital and Leeds General Infirmary. We provide high quality
acute hospital services for the population of Leeds and the surrounding area
and act as a regional centre for a number of specialist services such as
cancer and cardiac surgery. In total we employ around 13,000 staff across six
main sites, treating well over a million patients every year with an annual
budget of almost £1 billion.

The Trust is made up of the following hospitals:

Leeds General Infirmary
Chapel Allerton Hospital
Leeds Dental Institute
Seacroft Hospital
St James’s University Hospital
Wharfedale General Hospital


1.1    Leeds General Infirmary


Leeds General Infirmary is a specialist regional centre for a number of
complex conditions, along with providing many general acute hospital
services.

The Infirmary has one of the most diverse and architecturally contrasting sites
of any hospital in the country.

Its newest part is the Jubilee Building, opened in 1998, which houses a range
of services including an Accident and Emergency department, intensive care
facilities, state of the art operating theatres and a high-tech high dependency
unit. It also brings together some of the country's leading experts in caring for
patients with serious brain injuries and heart and lung problems.

The Yorkshire Heart Centre provides services for children and adults with
serious heart conditions and through the hospital's partnership with Leeds
University has trained some of the world's most renowned heart surgeons.

The centralisation of neurology in 2005 from St. James's University Hospital
means the LGI also continues to lead the way in brain surgery.

A helicopter deck on the Jubilee Building has seen thousands of flights by the
Yorkshire Air Ambulance bringing sick patients from all across the Yorkshire
region and beyond to be treated by experts at LGI.

Work started in 2009 to centralise children’s hospital services at LGI as part of
a major city-wide reconfiguration scheme involving investment of up to £35

million. In April and May 2010 saw the creation of the Leeds Children’s
Hospital, one of the largest facilities for sick children anywhere in the country.

1.2    Chapel Allerton Hospital


The present Chapel Allerton Hospital was                  opened in 1994 by HRH the
Duchess of Kent. A modern, purpose-built                 facility, it was one of the first
hospitals in the country to have stimulating             artwork included as an integral
part of the design, and provides a bright,                modern environment for staff,
patients and visitors

The new hospital was built on to the existing Newton Green Wing, dating from
the 1970s. It is located in pleasant grounds with ample car parking.
Chapel Allerton Hospital is one of the most popular facilities in the Trust with
our patients, receiving consistently positive feedback. Staff work very hard to
provide a friendly, caring environment.

The Chapel Allerton Orthopaedic Centre (CHOC) opened in the hospital in
early 2005 and is the Trust's dedicated centre for the diagnosis, management
and treatment of adult patients with upper and lower limb complaints.
Since it was established, the service has helped to dramatically reduce
waiting times for orthopaedic treatment and cut the length of stay following
inpatient treatment for joint replacements by half.

1.3    Leeds Dental Institute


The Leeds Dental Institute provides routine dental care for patients in support
of undergraduate training and also acts as a referral centre for specialist
dental care to the population of Leeds and the surrounding region. It also
provides education for dental undergraduate and postgraduate students and
dental auxilliary staff.

The Institute is situated on levels five and six of the Worsley Building on
Leeds University Campus. It shares this building with other University
Departments and the Medical and Dental Library.

1.4    Seacroft Hospital


Seacroft Hospital is located off York Road in the east of Leeds, and is one of
the oldest hospital sites in the city still in use.
It has seen considerable investment in recent times, notably with the opening
of the Leeds Reproductive Medicine Centre in January 2010, creating one of
the biggest such facilities anywhere in the NHS.

Significant developments in renal dialysis facilities have also taken place on
the site, which is gradually being modernised around a central core of key
services which serve local people from east Leeds and a wide surrounding
area.

Thanks to its accessible location, the hospital is well suited to providing a wide
range of day facilities.

Seacroft Hospital was opened in 1904, originally serving as an infectious
diseases hospital for the city. This early history explains the unusually large
size of the site and its rather curious layout, with wards located well away from
each other separated by long corridors.
The tall brick water tower is one of the best-known landmarks of the eastern
part of the city

The hospital site has evolved considerably over the last few years and it is
continuing to change.




1.5    St. James’s University Hospital


Affectionately referred to as “Jimmy's”, the building is generally regarded as
“Europe’s largest teaching hospital” has a world -renowned reputation as a
centre for medical research and education.

It is home to a number of specialist units providing care for patients from
across the Yorkshire region and beyond.
For example, St. James's houses the Yorkshire regional transplant service,
providing tertiary services for kidney transplants and is also a designated
national site for adult liver transplantation.

The St James’s Institute of Oncology, based in the new £220 million Bexley
Wing, was officially opened by HRH The Princess Royal in summer 2008.
This striking building provides a world-class service to patients from across
the region, and replaced outdated facilities formerly provided at Cookridge
Hospital.

In 2010 older people’s services and acute medicine were centralised at the
hospital, as part of a complex scheme which saw children’s hospital services
from St James’s moved across the city to join those at Leeds General
Infirmary.



1.6    Wharfedale Hospital


Wharfedale Hospital is one of the newest buildings in the Leeds Teaching
Hospitals NHS Trust, providing a modern, purpose-built environment where
excellent standards of patient care are delivered.

Led by the hospital’s Matron, Zoe Kirk, staff who work at the hospital have a
passionate attachment to it and a personal commitment to providing the best
possible care to the local community. They pride themselves on their friendly,
patient-centred service.

The Leeds Teaching Hospitals NHS Trust is committed to developing the role
of Wharfedale Hospital and increasing the numbers of patients who use it.
There has been a drive to bring additional services into the building where
those can be provided closer to people’s homes, to avoid local residents
having to travel to access services which can be safely delivered on site.

Recent examples of this include a local chemotherapy service added in 2009
and plans to develop breast screening services at the hospital during 2010.

Strong community links are in place with the proud market town of Otley,
where the hospital is located, as well as with neighbouring communities such
as Guiseley, Rawdon, Yeadon, Burley and Pool, all of whom feel a strong
attachment to “Wharfedale”.

While the current hospital is ultra-modern, it is the successor to a well-loved
former hospital with roots going back more than a century. The spirit of the old
hospital, with its strong track record of providing healthcare to generations of
local people, remains true to this day in its new surroundings.

3    SUMMARY OF REQUIREMENTS

Leeds Teaching Hospitals NHS Trust (the Trust) has a requirement to procure
Heart Valves both tissue and mechanical, Valve Conduits, Bands & Rings for
Valve Repair for use in Cardiac Theatres within the Trust and invites
interested suppliers to complete this Pre-Qualification Questionnaire (PQQ) as
their Expression of Interest.

As part of this tender process, it is essential that suppliers understand that the
Trust wishes to develop e-enabled managed services and processes that
makes the process of supply and managing stocks easier for clinical staff
within the Theatre Area.

The contract will be let for an initial period of two years with the option to
extend for a further period of one year up to a maximum contract length of
three years. This is dependent upon a satisfactory level of performance to be
monitored through review meetings and agreed Service Levels.

RETURNED AND ALL THE REQUESTED DOCUMENTS, IN ENGLISH,
SUPPLIED WITH NO EXCEPTIONS, UNLESS YOUR COMPANY ARE
REGISTERED WITH SID4HEALTH, IN WHICH CASE SECTION 5.1, 5.2.9,
5.2.10, 5.3, 5.4, 6, SCHEDULE 1 AND 2 MUST BE COMPLETED. IF
SECTION 5.2 IS NOT COMPLETED THE ONUS IS SOLELY WITH THE
COMPANY SUBMITTING THE PQQ TO ENSURE THE DETAILS
AVAILABLE ON SID4HEALTH ARE CURRENT AND CORRECT.

THE PQQ FORM AND ALL THE RELEVANT REQUESTED DOCUMENTS
MUST BE SUPPLIED VIA DUE NORTH PRIOR TO THE CLOSING DATE
AND TIME STATED IN SECTION 4.4.

BIDDERS WHO FAIL TO COMPLETE THIS PQQ IN FULL OR TO PROVIDE
ANY OF THE DOCUMENTS REQUESTED MAY HAVE THEIR
APPLICATIONS REJECTED AND MAY NOT BE CONSIDERED FOR
INCLUSION IN THE LIST OF ORGANISATIONS INVITED TO SUBMIT A
RESPONSE TO THE INVITATION TO TENDER (ITT).

4         SUMMARY OF PRE-QUALIFICATION PROCESS

This Pre-Qualification Questionnaire (PQQ) has been developed to allow bidders to be
evaluated in relation to their organisational capability, relevant experience, technical capacity
and ability to meet the specified requirements as set out above in section 3.

We will apply the following objective criteria to the selection process:

           Economic and Financial Standing - the bidder must be of a financial standing to
            participate in a procurement of this size.

           Track record - the bidder can demonstrate that it has delivered similar goods/services
            in similar circumstances to those detailed in the Summary of Requirements section
            above.

           Capacity and Capability - assessment of the totality of resources and core
            competences available to the organisation.

           Completeness and Compliance - full completion and submission in accordance with
            the requirements of the PQQ.


4.1       Scoring Matrix


Score Definition
             An exceptional response, indicating all the features, attributes behaviours and
      5      strengths over and above that you would normally expect from a winning bidder.

             A very good response, including all the features, attributes behaviours and
      4      strengths that you would expect from a winning bidder.

             A satisfactory response to the extent that it meets the required standards and
             provides a full response. This may not be the standard you expect from a winning

             bidder and have limitations in their response.

             Unsatisfactory response but has some merit.

             A poor response with little merit.

Submissions will be evaluated only on the basis of the information provided within your
response to this PQQ together with any supporting documentation requested.

The evaluation of submissions may result in the short listing of bidders who will move to the
ITT stage of the process.


4.2       Completion of Application (PQQ)

Please answer all questions (write N/A stating why they are not applicable where appropriate)
and ensure that all documents requested are clearly marked and submitted with your
completed PQQ.

Any information/documents submitted in response to this PQQ must relate to the bidding
organisation and not to other legal persons or organisation[s]. The bidder is the organisation
that you propose will enter into a formal contract with Leeds Teaching Hospitals NHS Trust for
the supply of goods or services.
