8. Prices shown herein are “net” ceiling prices-not to exceed rates. The net ceiling rates include
malpractice insurance for non-personal services task-orders and exclude travel and lodging. Personal
Services task-orders will also be accepted. See page 3 of this price list.

11. Medical Liability Insurance Limits: $1 million per occurrence; $3 million aggregate.
12. Ordering Procedures:
For services, the ordering procedures, information on Blanket Purchase Agreements (BPA’s), are found
in the Federal Acquisition Regulation (FAR) 8.405-3.
13. Background Investigations:
The following is the basic background investigation included in the NTE price:
Criminal background checks involve record reviews of all county court records where an individual has
lived within the last seven (7) years. The reviews include an investigation of malpractice history.
TechForce3 performs State specific Family Service Reviews.

14. Vaccination and Immunizations:
All vaccinations and immunizations are checked against physician notation, disease history, lab
documentation, and x-rays. A 10 panel urine drug analysis is performed testing for the following drugs:
cannabinoids/marijuana, cocaine, amphetamines, opiates, benzodiazepines, methadone, methaqualone,
barbiturates, PCP. Tests are performed annually.

Shift Differentials:
Work Week: Sunday (12:00am) through Saturday (11:59pm).
Overtime Rate(s):
All states but CA: The normal hourly rate +$10.00/hour (which is equivalent to 1.263x the normal hourly rate)
for any work in excess of 40 hours per week per state and local labor laws.
Overtime for California only: The normal hourly rate +$10.00/hour (which is equivalent to 1.263x the normal
hourly rate) after the first 8 hours worked in excess of 40 hours per week;
Double-time for California only: The normal hourly rate +$20.00/hour (which is equivalent to 1.527x the
normal hourly rate) for any work in excess of 12 hours in a day/40 hours per week and in accordance with state
and local labor laws.
Holiday Rate(s): hourly rate +$10.00/hour (equivalent to 1.263x the hourly rate) for days worked that are
federally recognized holidays

On-Call Rate(s): $5.00/hour; must be able to report to the facility within 30 minutes of notice; must be
available by beeper or cell phone. Provider must be able to provide phone consultation.
Call Back Rate(s): hourly rate +$10.00/hour (equivalent to 1.263x the hourly rate) with a two hour
minimum of work when called back; start time begins upon arrival at the facility
