The undersigned owners in the Capitol Hill Homeowners’ Association, Inc. appoint

2007 at the POOL HOUSE 4401 N.W 99th Way at 6:00 p.m. and any adjournment thereof.
(Please appoint a homeowner from the community that will attend the meeting to represent
you in order to establish a quorum. If this space is left blank, the President of the
Association will be appointed as your Proxy.)
This Proxy is for attendance purposes so as to be able to establish a quorum, to vote in favor of
or against the adoption of the Amended and Restated Declaration and By-Laws, and for such
other lawful business as may be conducted during the Special Member’s Meeting.
