Introduction
This workbook brings together information about mental health problems and services in
the area served by Newcastle PCT. It is intended to provide easy access to as much
background detail as possible about the current state of affairs to assist planners,
managers and clinicians in their work of ensuring that services are focussed as effectively
as possible on the needs of the people they are intended to serve. Information is drawn
from a range of sources; these are explained alongside the data.
The workbook is produced by the Centre for Public Mental Health (CPMH) at Durham
University, a venture funded originally by Health Authorities and NHS Trusts providing
mental health care in the former NHS Northern Region.
The workbook is currently divided into two sections:

The information available can only give a partial view of these issues. At present no
satisfactory data is available on service outcomes. However the workbook is not intended
to be static. The CPMH is working actively both with the mental health care NHS Trusts,
and in other areas to expand the scope and usability of the information.

1. Mental Health Care Needs.
Many people who suffer with mental health problems can benefit from a range of types of
care. Some seek help from their General Practitioners (GPs), some of these may be
referred to specialist services, some approach social services departments, some make
use of voluntary sector projects and some do not seek help at all, possibly unaware of its
existence. In planning care, it would be a helpful start to know how many mentally ill
people there are in the population as a whole. This section presents what is known about
this.
There are broadly two approaches to estimating the numbers of people with mental health
problems in the general population. One is relevant to the types of common problems,
such as depression and anxiety states, for which people commonly consult their GPs. The
other relates to the more severe types of problem, such as schizophrenia or manic-
depressive illness for which people commonly use specialist services.
Common mental health problems.
The first National Psychiatric Morbidity Survey [Meltzer, 1995 #4041] was a major national
study, involving interviews with nearly 10,000 people of working age (16-64) in 1993. This
showed that overall, 16% of people in the general population and living in private
households suffered with some type of neurotic disorder. In addition to this, 4 people per
thousand had suffered from some type of psychotic disorder, 47 people per thousand
suffered from alcohol dependence and 22 per thousand from drug dependence.
However these rates varied considerably between different groups within the population.
Unemployed people and those not seeking work were at higher risk of mental illness. The
commoner types of mental disorder tend to be found more frequently in people in their 30s
and 40s, while drug dependency was found more often in younger people. People living
alone, in single parent families or in rented accommodation were also at higher risk. Thus
the scale of mental health problems varies between areas in relation to the frequency of
these characteristics.
The methodology section of the Office of National Statistics undertook a detailed study of
these relationships to establish ways of predicting the actual rate of mental health
problems in any area [Heady, 1996 #1447]. Using census information about the frequency
of these and other characteristics in the 200 Postal Sectors on which the survey was
based, a mathematical model was developed to permit calculation of the probable rates of
any neurotic disorder and of depression in any area.
This section presents the results of this work for the electoral wards making up the
Newcastle PCT area. For easy reference, a map naming the wards in the PCT is
presented inside the cover (Map 1).
Map 2 and map 4 show the proportion of working age population estimated to experience
any neurotic disorders and depression respectively for the whole of the area or „sub-
region‟ covered by the CPMH. Maps 3 and 5 show the same data, but just for the
Newcastle PCT district. The shading on the local maps is the same as on the maps for the
sub-region in order to convey an impression not only of where problems are likely to be
more or less common within the PCT area, but also the frequency of problems within the
PCT relative to the area as a whole.

A specific commentary on the individual PCT here]
Tables 1 presents the same data in numerical form. This data has two obvious uses:
1. It may suggest appropriate deployment of primary care level mental health
resources within the PCT area.
2. It may be used as a „ready-reckoner‟ for calculating the likely numbers of people
with mental health problems in GP patient lists. For this purpose it is necessary to
know the numbers of patients of working age in each GP list, grouped by the
electoral ward in which they live. A spreadsheet set up with the ready-reckoner
calculations is available on the CPMH website.

More severe mental health problems.
The types of mental health problem commonly associated with use of specialist care show
a greater degree of concentration in areas of social deprivation [Smith, 1996
#3685][Jarman, 1992 #522][Thornicroft, 1991 #1409][Glover, 1999 #1026]. Population
based surveys, such as the National Psychiatric Morbidity Survey, do not identify
sufficiently large numbers of cases for the development of models such as those described
above. Modelling work for these problems is thus dependent on identifying patterns of use
of specialist services. At present the only type of service for which use is documented in
sufficient detail is in-patient admission. Models predicting admission rates for small areas
are thus used as the basis for allocating mental health care resources within the NHS
[Glover, 1999 #1151].
The model currently used by the Department of Health to allocate resources to English
Health Authorities was developed at the Centre for Health Economics at York University
[Carr-Hill, 1994 #1444] (the York Index). Unfortunately the use of the York Index at small
area level is impeded by the fact that its calculation requires data on standardised mortality
ratios; these are not published for areas smaller than local or health authorities. To
address this problem, Glover developed first a similar index, the Mental Illness Needs
Index (MINI) [Glover, 1998 #1168] and more recently a newer index (MINI2000) [Glover,
#4042], based on contemporary deprivation data published by the Department of
Transport, Local Government and the Regions
(http://www.urban.dtlr.gov.uk/research.htm).
Map 6 shows MINI2000 data for the whole area covered by the CPMH. This index shows
the ratio between the likely psychiatric admission rate for the area and the national
admission rate for working age adults. Map 7 shows the corresponding data for the area
served by the Newcastle PCT.
This data can be used to examine the appropriate distribution of resources within a PCT
area. To facilitate this, figures for each electoral ward in the PCT area are displayed in
table 2. This shows the ward population (working age adults) and the likely number of
admissions in a year arising from the population. It is suggested that this could be taken
as a working estimate of the proportion of the areas secondary care caseload likely to
come from the ward. These numbers may be grouped according to the organisation of
clinical sector team arrangements to provide a measure of likely relative workload. A
spreadsheet ready-reckoner is provided on the CPMH website to facilitate these
calculation.

2. Service provision
Mental health service mapping
To support the implementation of the mental health national service framework and enable
progress in the reconfiguration of mental health services to be monitored, the Department
of Health commissioned the Centre for Public Mental Health at Durham University to carry
out a comprehensive service mapping exercise in 2000. This built on work developed by
Di Barnes at the University in the field of social care mapping, by extending it to include all
mental health services in the statutory, voluntary and private sectors. Each National
Service Framework (NSF) Local Implementation Team (LIT) was asked to report all the
services available to the population they served.
In 2001 the exercise was developed further, both technically and geographically.
Technically, the process of data collection was transferred to the internet. The data
collection pages can be found at:

Geographically the structure of the data was enhanced by asking LITs to report the activity
of major service elements separately for „patches‟ served by only one PCT and only one
social services authority. This enables the data to be viewed from all three perspectives.
It is this development that enables the production of this report for services to PCTs.
The service mapping covers all mental health services in England. Updating forms part of
the NSF implementation assessment process, carried out each autumn. Completion is the
responsibility of the LIT Lead and is intended to reflect the situation at the end of
September. Data presented here reflects September 2001.

Services are listed by service type and are grouped into 13 categories: access services;
community mental health teams; clinical services; continuing care services; secure
services; psychological services; services for mentally ill offenders; other treatment
services; day services; home care services; carers‟ services; support services and
accommodation. Definitions of each service type have been specified and can be found
on the mapping website.
The accuracy of Service mapping data is not yet as good as could be wished. The
development of the service mapping database has been a huge undertaking, involving
hundreds of people all over England. Already within 2 years, it has gathered information
on over 16,000 services but the mechanics of the process are still in a developmental

phase. There are difficulties both in obtaining complete data (as the mapping requests
information that was not previously gathered) and in being sure that the data all relates to
services for working age adults (as scope and definitions of the exercise specify). Hence
analysis of the 2001 data (presented here) should be treated with caution. However, it
provides a useful starting point for consideration of service provision, gaps analysis and
benchmarking. It is anticipated that as the data becomes better known and more widely
used, and as data feedback and checking becomes more automated, accuracy will
increase.
The results of the 2001 mapping will be reported in the form of an Atlas of LIT areas but
additional tables can be run live on the CPMH website at:

PCT service provision
Newcastle PCT is located in the Newcastle LIT and a full list of LIT services can be found
on the mapping website. For the purposes of this profile, all services which are recorded
as supporting service users from the PCT area have been listed in the following pages.
Where a clear link has been made with the PCT, a selection of service details have been
given. In some cases, LITs have not provided sufficiently detailed data about some
services in their area for it to be clear which PCT(s) they relate to. In these cases the
existence of additional unassigned services within the LIT area is noted.
Staffing levels
Staff details are currently only requested in relation to major types of statutory service
provision. In some cases, where a service element, such as an intensive care unit or an
assertive outreach team provides care over an area wide enough to encompass more than
one PCT, there is a difficulty in identifying how many staff should be regarded as providing
a service to any individual PCT. Estimates have been made by apportioning the total staff
for these services to the several PCTs they serve on the basis of the relevant caseload
measure. Where this has been done, an * indicates that the level of staffing given is not
the full number of staff working within the service.
