      1.   Classified Employee of the Month for October 2009
           Mr. Bob Bilek, Director of Human Resources, will introduce Mr. Bob Shupe of Blythe Middle School,
           who has been selected as Classified Employee of the Month for October 2009. Mr. Shupe was selected
           by the Superintendent’s Advisory Committee (SAC) from nominations submitted district wide.

      2.      Student of the Month – Secondary Level (Grades 6-12)
              Mr. Marrio Walker, principal of Palo Verde Valley High School, will introduce Ashley Firquain who has
              been selected Secondary Student of the Month for October 2009. Miss Firquain is currently a senior at
              Palo Verde Valley High School.

H.    ADOPTION OF AGENDA

I.    REPORT ON ACTION TAKEN IN CLOSED SESSION
      The Board will report on action (if any) taken in closed session.

J.    HEARING SESSION
      This is an opportunity for community members to address the Board on agenda and non-agenda items. Those
      wishing to speak are to complete the form located on the back table and present it to the secretary prior to the
      meeting. Speakers are limited to three minutes each and each subject to twenty minutes except by consent of the
      Board of Trustees.

      Neither Board members nor staff members will respond during the public hearing. Speakers who ask questions
      will be informed by the President that responses may be referred to the Superintendent for investigation or
      resolution. Speaking time is limited to three minutes per speaker, twenty minutes total.

      If necessary answers will be given:
              a. by telephone after research
              b. by mail after research
              c. at the next regular meeting of the Board of Trustees as an agenda item

      The Board also would like to remind anyone who wishes to lodge a complaint against a District employee that
      there is an existing Board adopted policy in effect specifically for this purpose. This policy contains a
      comprehensive procedure for reviewing and investigating public complaints, which includes a right of appeal to
      the Board.

      This process provides a much more thorough opportunity for the complainant and the employee to be heard and
      the merits of the complaint to be properly investigated. Anyone who wishes to receive a copy of this policy may
      contact the Superintendent’s Office.

      1.   Second Reading and Final Approval Revised Board Policy 5141.52: Suicide Prevention
           The Superintendent's Office submits to the Board of Trustees for second reading and final approval the
           revised Board Policy 5141.52 Suicide Prevention. The policy is being revised per the recommendation of
           the California School Board Association. Exhibit: C

      2.     Second Reading and Final Approval New Board Policy 3110: Transfer of Funds
             The Business Services Department submits to the Board of Trustees for second reading and final approval
             the new board policy 3110 Transfer of Funds. The policy was recommended for adoption by the
             California School Board Association. Exhibit: D

      3.     Second Reading and Final Approval Revised Board Policy 4154: Health and Welfare Benefits
             The Business Services Department submits to the Board of Trustees for second reading and final approval
             revised policy 4154 Health and Welfare Benefits. The policy was revised per the recommendation of the
             California School Board Association. Exhibit: E

      4.     Second Reading and Final Approval Revised Board Policy 5131.62: Tobacco
             The Special Services submits to the Board of Trustees for second reading and final approval the revised
             board policy 5131.62 Tobacco. The policy is being revised per the recommendation of the California
             School Board Association. Exhibit: F

      5.     Second Reading and Final Approval Revised Board Policy 5021: Noncustodial Parents
             The Special Services submits to the Board of Trustees for second reading and final approval the revised
             board policy 5021 Noncustodial Parents. The policy is being revised per the recommendation of the
             California School Board Association. Exhibit: G

      6.     Second Reading and Final Approval Revised Job Description: Director of Human Resources
             The Personnel Services Department submits to the Board of Trustees for second reading and final
             approval revised job description Director, Human Resources. The purpose of the revision is to indicate
             schedule of pay and make corrections to job titles within the body of the description. Exhibit: H

      7.     Approval of Board of Trustees Meeting Date Change
             The Superintendent’s Office submits to the Board of Trustees for approval a change in date to the Regular
             Board Meeting scheduled for November 10, 2009. The requested change is from November 10, 2009 to
             November 17, 2009.

      8.     Approval of Service Agreement: Arriba Education
             The Curriculum & Instruction Department submits to the Board of Trustees for approval an agreement
             with Arriba Education to provide Supplementary Educational Services (SES) to eligible students at Felix
             J. Appleby Elementary, Ruth Brown Elementary and Blythe Middle School. These schools are Program
             Improvement Schools, Year 2 and above. The California Department of Education has set the Per Pupil
             Rate for Palo Verde Unified at $1,242.20 for 2009-2010, which is the amount of services each eligible
             student is allowed to receive. Forty-nine (49) students are enrolled to receive services from Arriba
             Education, which reflects a total cost to the district not to exceed $60,867.80. Title I funds will be used to
             facilitate the cost of this agreement. Exhibit: I

      9.     Approval of Service Agreement: Carney Educational Services
             The Curriculum & Instruction Department submits to the Board of Trustees for approval an agreement
             with Carney Educational Services to provide Supplementary Educational Services (SES) to eligible
             students at Felix J. Appleby Elementary, Ruth Brown Elementary and Blythe Middle School. These
             schools are Program Improvement Schools, Year 2 and above. The California Department of Education
             has set the Per Pupil Rate for Palo Verde Unified at $1,242.20 for 2009-2010, which is the amount of
             services each eligible student is allowed to receive. Twenty-eight students are enrolled to receive
             services from Carney Educational Services, which reflects a total cost to the district not to exceed
             $34,781.60. Title I funds will be used to facilitate the cost of this agreement. Exhibit: J

      10.    Approval of Service Agreement: Avanza Online
             The Curriculum & Instruction Department submits to the Board of Trustees for approval an agreement
             with Avanza Online to provide Supplementary Educational Services (SES) to eligible students at Felix J.
             Appleby Elementary, Ruth Brown Elementary and Blythe Middle School. These schools are Program
             Improvement Schools, Year 2 and above. The California Department of Education has set the Per Pupil
             Rate for Palo Verde Unified at $1,242.20 for 2009-2010, which is the amount of services each eligible
             student is allowed to receive. Twenty-four (24) students are enrolled to receive services from Avanza
             Online, which reflects a total cost to the district not to exceed $29,812.80. Title I funds will be used to
             facilitate the cost of this agreement. Exhibit: K

      11.    CSEA Chapter 111 Contract Proposal
             CSEA Chapter 111 submits to the Board of Trustees an initial proposal for collective bargaining. In
             accordance with Government Code 3547, the Palo Verde Unified School District is presenting the
             proposal for public comment. The initial proposal of the exclusive representation will be brought back to
             the Board and presented for a public hearing and final public comment. A copy of the CSEA Chapter 111
             initial proposal has been placed on the back table of the board room for public review.

M.    CONSENT ITEMS
      1.   Minutes
           The Superintendent’s Office submits for the Board of Trustees’ approval the minutes from the Regular
           Meeting on October 6, 2009, Special Meeting on October 7, 2009 and amended Special Meeting on June
           4, 2009. Exhibit: A

      2.     Personnel Report #2009-10-07
             The Personnel Department submits for the Board of Trustees’ approval the current Personnel Report.
             Exhibit: B

      3.     Evaluation of the District Wellness Policy
             The Nutrition Services Department submits to the Board of Trustees for review a written evaluation of the
             District Wellness Policy for 2008-2009. An annual evaluation is a requirement of the District Wellness
             Policy. Exhibit: M

      4.     Ratification of Blythe Middle School’s Student Body Statement
             Blythe Middle School submits for ratification from the Board of Trustees their Student Body
             Statement for September 2009. Exhibit: N

      5.     RCOE Certification of Signatures
             The Personnel Services Department submits to the Board of Trustees for approval Riverside County of
             Education Certification of Signatures. The purpose of this document is to Authorize the Director of
             Human Resources as signatory on the Notice of Employment forms to be submitted to Riverside County
             of Education. An authorized signature is required for County approval on each new employee to the

             The Business Department submits to the Board of Trustee for approval resolution 2009-34: Exhibit: P

                     Res 3324 ARRA IDEA, Preschool Local Entitlement 09/10 Award decrease $59.00.
                     Res 3319 ARRA IDEA, Special Education Preschool 09/10 Grant decrease $36.00.
                     Res 3313 ARRA IDEA, Local Assistance Funding 09/10 Award decrease $2,843.00.
                     Res 0827 Administrator Training Program Funding 08/09-09/10 Award decrease $919.37.
                     Res 7010 Agricultural Vocational Ed Incentive 09/10 Grant increase $914.00
                     Res 3550 Carl D. Perkins Career and Technical Education Improvement decrease $1,753.00

      7.     Approval of Consulting Contract: Dolinka Group
             The Business Department submits to the Board of Trustees for approval a contract with Dolinka Group,
             for consulting services to assist PVUSD with Developer Fee Study for Level 1 funding as our previous
             study expired July 2009. This agreement is a multi-year contract to update our developer fee funding
             levels every two years when the State revisits its state approved funding levels. The contract is for $4,000
             in 2009, 2012 and 2014. Should the funding levels go up in 2010, Dolinka will re-do our study in 2010
             for $250. This contract will bring us back into compliance. Exhibit: Q

      8.     Ratification of Resolution 2009-26: Pupil Textbook and Instructional Materials Compliance
             The Curriculum and Instruction Department submits to the Board of Trustees for ratification resolution
             2009-26 Pupil Textbook and Instructional Materials Compliance. The public hearing for this resolution
             was held at the Regular Board Meeting held on October 6, 2009 which provided details to the extent to
             which textbooks and instructional materials were provided to all students, including English learners, in

      9.     Purchase Order Listing September 2009
             The Business Department submits to the Board of Trustees the Purchase Order Listing for September
             2009. Fund: Various. Fiscal Impact: $ 2,034,661.80 Exhibit: S

      10.    Ratification of Palo Verde Valley High School Student Body Statement
             Palo Verde Valley High School submits for ratification from the Board of Trustees their Student Body
             Statement for September 2009. Exhibit: T

      11.    Approval of Service Agreement: Basic Educational Services Team (BEST)
             The Curriculum & Instruction Department submits to the Board of Trustees for approval an agreement
             with Basic Educational Services Team (BEST) to provide Supplementary Educational Services (SES) to
             eligible students at Felix J. Appleby Elementary, Ruth Brown Elementary and Blythe Middle School.
             These schools are Program Improvement Schools, Year 2 and above. The California Department of
             Education has set the Per Pupil Rate for Palo Verde Unified at $1,242.20 for 2009-2010, which is the
             amount of services each eligible student is allowed to receive. Three (3) students are enrolled to receive
             services from BEST, which reflects a total cost to the district not to exceed $3,726.60. Title I funds will
             be used to facilitate the cost of this agreement. Exhibit: U

      12.    Approval of Service Agreement: Academic Tutoring Services
             The Curriculum & Instruction Department submits to the Board of Trustees for approval an agreement
             with Academic Tutoring Services to provide Supplementary Educational Services (SES) to eligible
             students at Felix J. Appleby Elementary, Ruth Brown Elementary and Blythe Middle School. These
             schools are Program Improvement Schools, Year 2 and above. The California Department of Education
             has set the Per Pupil Rate for Palo Verde Unified at $1,242.20 for 2009-2010, which is the amount of
             services each eligible student is allowed to receive. Eight (8) students are enrolled to receive services
             from Academic Tutoring Services, which reflects a total cost to the district not to exceed $9,937.60. Title
             I funds will be used to facilitate the cost of this agreement. Exhibit: L

                   Blythe Middle School submits to the Board of Trustees for permission to surplus textbooks and other
                   library books from the library which have become outdated and are beyond repair. Items will be
