My child has permission to perform in Cookeville Children’s Theatre production of
THE WIZARD OF OZ. I understand that he/she is expected to be on time to all
rehearsals and that he/she must notify the director by phone or letter if he/she will
miss a rehearsal. Cookeville Children’s Theatre also has my permission to use my
child’s picture or voice to advertise this production.

Most of the cast will play multiple roles. This show demands a dedicated group of actors so
unless you have made arrangements with the director by May 7th, attendance to all
rehearsals and dance workshops is mandatory.
