POLICY



Staff will form a partnership with families. This partnership is on going and based on

mutual trust. It focuses on family strengths and builds upon them by setting realistic goals

developed by family members. Needs identified will also be addressed throughout the

program year by providing support, information and referral. Home visits are

encouraged, and a minimum of three will be conducted in the home.





PROCEDURE



1. Family Advocates will initiate the Family Partnership Plan process (See Family 07-

Family Partnership Plan). The length of visit is a minimum of 60 minutes. The

Parent/Program Partnership form will be reviewed and integrated into the goal and

development phase of partnership building. The geno/gram and eco map form will be

initiated and completed on this first home visit.



2. Family Advocates will review the family=s file prior to the family visit so they can be

familiar with the information already shared by the parent or agency that referred the

family.



3. In no case will home visits be a condition of the child's enrollment in Head Start. If a

parent refuses home visits or there are repeated cancellations, Family Advocates will

consult with the Family Service Manager.



4. Families with the highest emergency needs will be scheduled first. The Strengths/Needs

form on HSFIS will outline strengths and needs. The Screening Criteria will be utilized to

assess needs and direct follow up activities.



5. In areas where the family has identified needs, the Family Advocate will utilize the Guide

to Community Resources to make appropriate referrals. These will be noted on the FPP

form.

6. The Child Abuse and Neglect Handbook, and Hip on Health handbook will be reviewed

with the parent(s) and/or guardian(s). A copy will be given to the parent(s) and/or

guardian(s).

7. Two surveys will be initiated and completed with families: one is the Family Service

Outcomes; the second is Second Step.



8. The Family Advocate will discuss the Family Partnership Agreement process and proceed

with establishing goals depending on parent readiness to do so. The Family Partnership

Agreement form will be completed and a copy will be given to the family.



9. Family Advocates will check on status of medical and dental exams. All such efforts will

be documented on Activity Services Delivery Tracking of HSFIS. Barriers will be

addressed with the Health Manager.



10. After the completion of the family visit, the Family Partnership Agreement will be filed

on a monthly basis. Genogram/Ecomap will be kept in the Family Advocate file.



11. If the family has a pre-existing plan, Family Service staff will collaborate with the

agency. The family and Pocatello/Chubbuck Head Start will sign a release of information.



12. A meeting will be scheduled to discuss the pre-existing plan with the Teaching Team,

family and agency. Service coordination will continue with the Teaching Team.



13. The Family Service Outcome survey will be initiated and completed with the family. This

survey will be reviewed by the Family Advocate to guide service delivery. After the completion

of Center Base #1 visit, the surveys will be given to the support services assistant.



14. The second step survey will be initiated and completed on this visit. After the completion

of Center Base #1 visit, the surveys will be given to the support services assistant.
