Tools for Evaluation of Your EYSC Project

Tool # 5: Project Outcomes Instrument

Purpose of Tool

This tool is designed to measure the impact of participating in the EYSC project on the community via

observations from selected teens, adult leaders, adult participants, and parents of youth participants at

the local sites.

Using this Tool

1. Identify the subjects for the study. For this instrument, select ONE EACH of the following people

from EACH of the local sites participating in the project: one adult from the local leadership

team, one teen from the local leadership team, one adult who has participated in the project

(not on the leadership team), and one parent of a youth participant (not on the leadership

team). (For each local site, FOUR people total to complete the instrument.)

2. For those under 18 years of age, obtain permission from their parents for them to participate in

the study. Examples of a cover letter, parental consent form, and minor assent form are

attached. The forms will need to be completed prior to asking minors to complete the

instrument.

3. Whenever the implementation plans for EYSC9 have been completed but NO LATER than

December, 2011, have the subjects complete the instrument at one of the group meetings.

(Alternatively, mail the instrument to the selected participants and have them return completed

instruments to the state leadership team.)

4. Enter the information from the survey into the database at http://eci.ext.msstate.edu Once you

are there, log in to the system and select “Enter data for one initiative” and select “Rural Youth

Development (EYSC9-New Projects or EYSC-9 Renew Projects)”
