The donor sent cash for the purpose of donating to the Iowa
Gambling Treatment Program.
CDC Sexual Violence Prevention & Education Program $ 183,195 1/17/08
This is a second (revised) notice of award for program activities
conducted between November 1, 2007 and October 31, 2008.
The grant number is 5VF1/CE001154-02. This report reflects the
remaining amount of funds to be received during year 02 of the
project period, which is from 11/01/06 through 10/31/2011.
DHS-CDC Implementing EHS Net in Iowa $ 150,000 1/29/08
These funds will be used to carry out the Environmental Health
Specialist Network project. These funds will allow us to improve
food safety in Iowa and continue collaboration with CDC, Cerro
Gordo Dept. of Public Health, IDIA and University Hygienic Lab.
SYNECTICS State Outcomes Measurement & Management System $ 150,000 1/30/08
To cover submission of substance abuse client data for treatment
admissions and discharges that occur during CY2008.
