INSTRUCTIONS:
1. Once a participant enrolls, type the MTN-015 PTID,
the Parent Protocol (example: MTN-003),
the Seroconversion Date, and the MTN-015 Enrollment
Date. The partipant's non-ART visit calendar will be
created.
2. Print the calendar. Hand record your initials in "Staff
Initials". For each visit where the 015 Enrollment
Date is after the "Window Closes" date, as well
as the visit window that contains the 015
Enrollment date, write "Missed" for "Acutal Visit
Date". Complete a Missed Visit CRF for each missed
visit, marking "visit not required per protocol" as the
reason for the missed visit. Print and place calendar i
in the participant's MTN-015 study notebook.
3. As the participant completes MTN-015 non-ART study
visits, hand-record the date of the visit in the "Actual
Visit Date" field. If a parent protocol visit was
conducted on the same day, hand-record the Visit
Code of the parent protocol visit in the appropriate
column. If a parent protocol visit was not
completed, hand record "no" or "N/D" in this field.
If the participant begins ART therapy:
Generate a new MTN-015 visit schedule for the
participant using the "ART-Calendar" tool (see "ART
Calender" worksheet in this file. Discontinue use of the
non-ART participant calender generated for the
participant.
