Hematology/Oncology (Adult and Aplastic anemia Irradiated
Pediatric) CMV-seronegative, if recipient is CMV- seronegative
Congenital hemolytic anemias Irradiated and CMV-seronegative not required routinely
Leukemia/ lymphoma receiving intensive chemotherapy; Hodgkin Irradiated
disease (treated or not) CMV-seronegative, if recipient is CMV- seronegative
Patients treated with Fludarabine or other purine analogs Irradiated

Rapid large volume (intraoperative) transfusions for children < 5 First 10 RBCs ordered: < 5 days old or washed

CMV-seronegative, if recipient is CMV- seronegative
In addition to standard requirement of leukocyte-reduction, specific subsets of patients may receive CMV-seronegative components until CMV antibody test result is available.
These requirements have been requested by GUH attending physicians because of the likelihood of severe immunosuppression or intense immunosuppressive therapy.
If CMV-seronegative RBCs or Platelets are not available, the Blood Bank will supply Leukocyte-Reduced components.
