REFERENCE CHECK AND BACKGROUND INVESTIGATION RELEASE
I understand that Perfect Search; Inc. may conduct reference checks and background
investigations. This may include information regarding character, work record, general
knowledge, and capabilities and reputation.
Consent to Conduct Background Investigation: I authorize Perfect Search, Inc. or its
agent to investigate my education and employment history. I understand that this
background investigation will include, but not be limited to, verification of all
information on this application, as well as interviews with past employees.
Consent To Conduct Criminal and Credit Background Investigation: I authorize
Perfect Search, Inc. or its agent to conduct a criminal and/or credit background
investigation. I understand that this background investigation will be conducted only if
requested by a client that is considering me for employment.
Consent To Contact Past Employers and Coworkers as References: I authorize
Perfect Search, Inc. or its agents to contact all past employers listed on my resume
(except those specifically excluded in writing below) and coworkers I have provided for
references. I further give permission to all current or previous employers, managers,
supervisors and/or coworkers to discuss my relevant personal and employment history
with Perfect Search, Inc. or its agents and to the release of such information orally or in
writing.
