					                                     CAS Forms
The IBO trusts that schools will implement systems to help students plan and organize
their time as well as maintain the integrity of the CAS programme. Each year the IBO
may request the submission of student portfolios for review. It is essential that students
complete all forms and maintain documentation of their activities

The following is a list of CAS forms students will use to plan, log, and report their CAS
projects. All forms can be downloaded from the IB page of the school’s website.

Every student will need to complete one:
  • Skill & Interest Inventory
  • CAS 2 Year Plan

Each project will have its own:
  • Project Proposal
  • Activity/Project Self Evaluation Form
  • Project Evidence (there is no form for this, please see explanation below)

In Addition:
  • Quarterly CAS Reports
     Students will complete this at the end of each quarter & submit it to the
     CAS coordinator

  • CAS: Student Final Summary
     Students will complete this form at the at the end of their CAS programme and
     include it in their CAS Portfolio (see next page of this handbook for Portfolio
     directions)




                                   Project Evidence
As part of each CAS project or activity students should keep tangible evidence of their
participation. The evidence should be submitted along with the project proposal and
project self-evaluation form at the end of each quarter.

The following is a list of ideas

       •   Pictures from the project
       •   Journal entries during the project
       •   Business cards from project leaders
       •   Planning documents from a project
       •   Emails regarding project planning or progress
       •   Awards or letters of recognition

                              CAS Portfolio

• Every IB Diploma candidate will submit a CAS Portfolio at the end of 3rd
  Quarter senior year (prior to Spring Break).

• The Portfolio should be a collection of planning, work, and reflection on
  the students’ 2 year CAS experience.

• The CAS Portfolio should be submitted in a 3 ring binder and be
  organized in the following way. All CAS forms are available to download
  from the school’s website.

• Please see the CAS coordinator for clarification.
