A) Tell us about your organization

Describe the vision of your organization in the context of the initiative in a 1200 word
narrative. Your description should briefly discuss among others, the following points.

B) Essential criteria
PLEASE NOTE: Your organization must be ready to agree to comply with all essential
criteria. Please, check all criteria to which your organization already complies.

Appendix B: Organization Profile Form
A) Organization Profile
Describe your experience in taking part in similar projects, projects of similar reach or
your present capacity to take part. If applicable, demonstrate the way your best practices,
your expertise or lessons learned from past experience could be used within the context
of the present initiative

B) Advantages of the initiative and required support
How have you established the advantages that this initiative will have for your
organization? Will it fulfill a particular need? Will it meet certain priorities? Please report
on consultations, studies or surveys done on this matter, if applicable.
Indicate if the success of the initiative in the context of the operations of your organization
rests on the support or authorization of other parties, including the support of the Canada
School of Public Service. Describe the type of support required and to what extent you
feel that this support would be necessary for the achievement of the objectives.
C) Targeted students
Briefly describe the group of students that you plan to target for the initiative. For
example, indicate the level, the study program(s) or any other information necessary to
describe the type of cohort.

Describe the means proposed to best communicate with the target group and how you
will be able to help the project team maintain a constant dialog with them for the duration
of the initiative.
D) Organizational objectives
Make a list of the objectives that you are trying to reach through this initiative. Objectives
are precise statements of what needs to be done to achieve expected results. Try to limit
their number to five at the most.
E) Project Plan: Main Phases
Make a list of planned main phases to achieve the objectives of the project other than the
ones stated in the logic model of the initiative (Guidelines for Application, page 9). Most
projects should include no more than five to eight activities or main phases. You will later
be asked to provide more detailed tasks within the Action Plan (Appendix C). Please note
that phases must complement or expand those already established within the context of
the initiative.
F) Products to be created or other output
Describe the output or products that will be created as a direct result of your activities
within the context of the initiative. It is not limited to material products. It could be for
example, activities, workshops, etc.

I) Anticipated Long-Term Impact and Sustainability
Indicate the impact that the initiative is anticipated to have beyond its duration. While
long-term impacts are not generally measurable within the life of the project, they should
still relate to the project’s primary purpose. You may also wish to use this space to
