REGISTER TO VOTE
To register to vote, you must provide a valid Wisconsin Driver’s License with your
current address. If you are not issued a Wisconsin Driver’s License, a Wisconsin ID
Card will be acceptable or the last 4 digits of your social security number. You can
register to vote two ways: in the Clerk’s Office or by mail. You can register to vote
in the Clerk’s Office during office hours. If you wish to register by mail, you can call
the Clerk’s office to have a Voter Registration application mailed to you or go to
http://elections.state.wi.us/docview.asp?docid=13109&locid=47 to download the
application. Please note if you register by mail, you MUST include a copy of your
Driver’s License. If you do not, your application will not be processed and you will
not be able to vote until you show ID, either at the polls or in the Clerk’s Office.
PERMANENT ABSENTEE BALLOTS
If you are indefinitely confined because of age, illness, infirmity or disability, you
may request a Permanent Absentee Ballot be sent to you for every subsequent
election until you are no longer confined or fail to return a ballot for an election.
Again, to have an Absentee Ballot sent to you, you must be a registered voter in the
Town of Waterford. If you are not sure if you are registered, please call the Clerk’s
Office at 534-2350.
If you are a registered voter, complete, sign and mail the attached Absentee Ballot
application to the following address. Please note, if you qualify to be a Permanent
Voter, please indicate by filling in the 2nd circle in section 5 of the application,
write “PERMANENT VOTER” on the top of the form and mail it to the following

Onceyour application has been verified, you will be sent an Absentee Ballot for all
2008 elections. In the fall of 2008, the Clerk’s Office will send you a letter asking if
you want to renew your calendar year Absentee Ballot application for 2009. If you
want to continue, you would fill out another form. That process will continue until
you no longer wish to receive Absentee Ballots.
