management company, ensure the applicable lease provisions permit such notice to be sent
by a management company, and if so, revise to state: “We are the management company
for the property located at ____ [Instruction: Insert property address.] and are sending this
notice at landlord’s request.] Pursuant to ____ [Instruction: Insert applicable lease
provision(s) detailing rent increases and notice.] of that certain written lease agreement (the

Landlord’s name.] as landlord (“Landlord”), and ____ [Instruction: Insert Tenant’s name.] as
tenant (the “Tenant”), this letter shall serve as notice of an increase of $ __________
