This Reasonable Accommodation Request Letter should be used
by any person desiring an accommodation for a disability under
federal law. The user can easily adapt this letter to fit their
specific situation. This agreement, also, includes crucial terms
related to keeping the letter confidential.

This letter serves as my request for a reasonable accommodation under the Fair Housing
Amendments Act of 1988, as amended (42 U.S.C. §§ 3601, et. seq.) for your apartment complex.
Due to my disability, I require the following accommodations:
