					This Code of Conduct can be used by any sports or youth organization and governs the
conduct of participants, participants’ parents or guardians, and guests while participating
in or attending an event. The participants and parents/guardians pledge to be
responsible for their words and actions while attending, coaching, or participating and
further confirm that their behavior will follow the listed code of conduct. This document
is intended to provide language for a standard code of conduct but can be customized
to fit the needs of any institution putting on an event or competition.
                                               CODE OF CONDUCT

This Agreement is between [Insert name of entity of institution] (“Institution”) and [Insert name
of participating member or individual] (“Participant”) and [Insert name of parent or guardian of
Participant] (“Parent” or “Guardian”) acknowledging their receipt and acknowledgement of this
Code of Conduct (“Code”).

We hereby pledge to be responsible for our words and actions while attending, coaching or
participating in any sport event and shall confirm our behavior to the following Code:

    1.            We will not engage in unsportsmanlike conduct with any coach, parent, player,
              participant, official or any other attendee.
    2.            We will not encourage our child, or any other person, to engage in
              unsportsmanlike conduct with any coach, parent, player, participant, official or any
              other attendee.
    3.           We will not engage in any behavior which would endanger the health, safety or
              well-being of any coach, parent, player, participant, official or any other attendee.
    4.            We will not encourage our child, or any other person, to engage in any behavior
              which would endanger the health, safety or well-being of any coach, parent, player,
              participant, official or any other attendee.
    5.           We will not use drugs or alcohol while at a youth sports event and will not attend,
              coach, officiate or participate in a youth sports event while under the influence of
              drugs or alcohol.
    6.            We will not permit our child, or encourage any other person, to use drugs or
              alcohol at a youth sports event and will not permit my child, or encourage any other
              person, to attend, coach, officiate or participate in a youth sports event while under
              the influence of drugs or alcohol.
    7.             We will not engage in the use of profanity.
    8.            We will not encourage our child, or any other person, to engage in the use of
              profanity.
    9.            We will treat any coach, parent, player, participant, official or any other attendee
              with respect regardless of race, creed, color, national origin, sex, sexual orientation or
              ability.
    10.           We will encourage our child to treat any coach, parent, player, participant, official
              or any other attendee with respect regardless of race, creed, color, national origin, sex,
              sexual orientation or ability.
    11.           We will not engage in verbal or physical threats or abuse aimed at any coach,
              parent, player, participant, official or any other attendee.
    12.          We will not encourage our child, or any other person, to engage in verbal or
              physical threats or abuse aimed at any coach, parent, player, participant, official or
              any other attendee.

    13.           We will not initiate a fight or scuffle with any coach, parent, player, participant,
              official of any other attendee.
    14.          We will not encourage our child, or any other person, to initiate a fight or scuffle
              with any coach, parent, player, participant, official or any other attendee.
    15.            We will abide by any decision rendered by Institution.
    16.           We will be responsible for the behavior of all those attending an event on our
              child’s behalf.
