Transcript Request Process and College Application Procedures 1) Submit a Completed Transcript Release Form to the Transcript Office. This form must be signed by both you and a parent/guardian and submitted before any records can be released to a college, employer or scholarship organization. Submit your Transcript Request Forms according to the following deadlines. Any forms submitted beyond these deadlines will incur a $5 late fee in addition to the regular transcript fees. (The first three College Transcripts and the first three Scholarship Transcripts are free. For every transcript after three in each category, there is a $5 charge per transcript payable in advance. Transcripts will NOT be sent until the fee is paid.)

If applying to a college with an October 1, 2009 deadline, Transcript/Rec. deadline is September 21, 2009. SCHOLARSHIP TRANSCRIPTS: Students are highly encouraged to look at the scholarship application requirements to determine whether or not an official transcript is required. In cases where an unofficial transcript is acceptable, there is no fee required. 3) Test Scores It is the student’s responsibility to have all test scores sent directly to each college. SATs can be sent via www.collegeboard.com and ACTS can be sent via www.actstudent.org. 4) Letters of Recommendation • If you need a counselor recommendation, you should also submit a Recommendation Letter Request and Senior Information Profile within 30 days of the application deadline. (The Profile needs to only be completed once.) • If you need a teacher recommendation, you should check with him/her for separate requirements. I have read and understand the Transcript Request Process and College Application Procedures. I hereby authorize Chantilly High School to release any information that is requested from my cumulative record by any colleges, employers or scholarship agencies. I also give permission for my 7th semester grade report and Mid-Year Report to be mailed to any colleges to which I apply in early February. During the college search and application process, colleges often ask students or counselors whether or not applicants have been found responsible for a disciplinary violation at an educational institution attended from 9th grade (or the international equivalent) forward, whether related to academic misconduct or behavior misconduct, that resulted in probation, suspension, removal, dismissal or expulsion from the institution. We expect students to answer this question honestly and openly and expect that any information will be shared with colleges upon submission of the application, while the application is being reviewed and after an admissions decision has been made.
