                             EXAMINING BOARD

                                         NOVEMBER 23, 2004

The meeting was called to order by David J. Waskowicz, Acting Chairman, at 9:30 A.M.
in Room 117 of the State Office Building, 165 Capitol Avenue, Hartford, Connecticut.

REVIEW MINUTES OF PREVIOUS MEETINGS

The Board voted unanimously to approve the September 28, 2004 minutes, with amendments to include
Mr. Livingston’s comments regarding Massachusetts not recognizing a Connecticut licensee to sit for
the examination in Massachusetts; the Board acknowledging that Continuing Education Units are
applicable to Electrical and Plumbing Work licensees and not applicable to Fire Protection Sprinkler
Work licensees; and conditional acceptance of a Stipulated Agreement relating to the Formal
Administrative Hearing in the Matter of Bell Fire Extinguisher Co., Inc., consisting of a Cease and
Desist order and a Civil Penalty in the amount of $20,000.00, of which $10,000.00 has been received to
date. The remaining $10,000.00 will be delivered to DCP via Federal Express. Upon receipt of the
entire $20,000.00, DCP will provide the Board with the Stipulated Agreement for acceptance and
signature.

Mark A. Turcotte, Westfield, MA – F-1 Application. – Approved
The Board voted unanimously to approve Mr. Turcotte’s application.

Jeffrey J. Galovich, Enfield, CT – F-2 Application. – Denied
The Board voted unanimously to deny Mr. Galovich’s application due to lack of required
letter from the Connecticut Department of Labor relating to completion of a bona fide Fire Protection
Sprinkler Work apprenticeship training program.

Joaquin Riva, Fort Lauderdale, FL – F-2 Application. – Approved
The Board voted unanimously to approve Mr. Riva’s application.

Kevin M. Roberts, Apex, NC – F-2 Application. – Approved
The Board voted unanimously to approve Mr. Roberts’ application.

Joshua G. Tippett, Fairfield, CT – F-2 Application. – Denied
The Board voted unanimously to deny Mr. Tippett’s application due to lack of required
letter from the Connecticut Department of Labor relating to completion of a bona fide Fire Protection
Sprinkler Work apprenticeship training program.

David A. Dal Pra, Mount Vernon, NY – F-4 Application. – Denied
The Board voted unanimously to deny Mr. Dal Pra’s application due to lack of required
letter from the Connecticut Department of Labor relating to completion of a bona fide Fire Protection
Sprinkler Work apprenticeship training program.

RECONSIDERATION OF PREVIOUSLY SUBMITTED APPLICATIONS

Richard A. Marshall, Columbia, CT – F-2 Application. - Denied
The Board denied Mr. Marshall’s application at their 5/25/04 meeting due to lack of
required occupational and school instructional experience. After further review of
Mr. Marshall’s application, the Board voted unanimously to deny his application due
to lack of required occupational and school instructional experience. No additional new
information was provided for review at today’s meeting.

Yuriy Rudshteyn, Staten Island, NY – F-3 Application. - Denied
The Board denied Mr. Rudshteyn’s application at their 7/27/04 meeting due to
lack of a required F-4 (Journeyperson) license for a minimum of two years.
After further review of Mr. Rudshteyn’s application, the Board voted unanimously
to deny his application due to lack of a required F-4 (Journeyperson) license for a
minimum of two years.


LATE ARRIVING APPLICATIONS

The Board voted unanimously to add the following applications for review:

Mark Dumschott, Naugatuck, CT – F-2 Application. – Approved
The Board voted unanimously to approve Mr. Dumschott’s application.

Ernest F. Stokes, III, South Hadley, MA – F-2 Application. – Approved
The Board voted unanimously to approve Mr. Stokes’ application.

FORMAL ADMINISTRATIVE HEARINGS

In the Matter of David Bemis             Docket No. 04-1005

Pursuant to the recommendation of DCP Counsel, the Board voted unanimously to dismiss the
charges on this matter.

In the Matter of Mark Corcoran           Docket No. 04-1006

Mr. Corcoran requested a continuance on this matter due to a schedule conflict that precluded him
from appearing at today’s meeting. The Board voted (4-3) to have this matter continued and
requested that Mr. Corcoran appear before the Board on the date this matter is to be continued.

In the Matter of Bell Fire Extinguisher Co., Inc.

This matter was the subject of a Formal Hearing held 9/28/04, at which time the Board conditionally
accepted a Stipulated Agreement consisting of a Cease and Desist order and a Civil Penalty in the
amount of $20,000.00, of which $10,000.00 has been received to date. The remaining $10,000.00 will
be delivered to DCP via Federal Express. Upon receipt of the $20,000.00, DCP will provide the Board
with the Stipulated Agreement for acceptance and signature.


CORRESPONDENCE

Leslie Joplin, Educational Programs Coordinator – Correspondence dated 5/13/04, regarding
AFSA Apprenticeship Training Course.

Barbara, Gould Institute – Correspondence dated 10/1/04, regarding Sprinkler Fitting Contren
Learning Series.

Joshua Tippett – Consumer Complaint Statement dated 10/4/04, regarding administration of
Connecticut’s Apprenticeship System.


OLD BUSINESS

PSI examination results – The Board has requested PSI examination results dating back to
February 2004 which have not been made available to date. The Board is requesting that this
information be made available for review at their next meeting scheduled for January 24, 2005.


NEW BUSINESS

PSI open-book examination – The Board discussed the issue of open-book examinations at PSI,
with an emphasis on what materials are allowed into the examination station as open-book.
The Board feels there is a need for establishing what materials qualify as open-book material.
