To Parents of Christ King School and Religious Education students entering 5th,
6th, 7th or 8th grade for the 2010-2011 school year

Important Dates to Remember:

May 17, 2010: Volleyball registration begins at 6:30pm prior to the Athletic
Committee General Meeting

June 5, 2010: Volleyball registration closes at midnight – NO REGISTRATION
FORMS WILL BE ACCEPTED AFTER THIS DATE/TIME!

August 15, 2010: Open Gym at CK for 5th & 6th graders: 4-5:30pm
                                    7th & 8th graders: 5:30-7pm

August 16-17: Player Evaluations

Volleyball Registration

Attached you will find the following forms: Sport Sign-Up Form, Risk
Acknowledgement & Consent to Participate Form, Physical Examination Report
Form, Student Athlete-Medical Information & Emergency Consent Form, Code of
Conduct for Players & Parents, Student-Athlete Sportsmanship Pledge and
Parent/Guardian Sportsmanship Pledge- seven total forms.

All forms need to be completed and turned in (along with the fee) when you
register your child for volleyball. The only exception is the Physical Examination
Report. If your child is new to a CK sports program, a physical examination,
completed by a doctor after April 1, 2009, will be required for your child to
participate in CK athletics. If your child has a current Physical Examination
Report on file with Kathryn Devine, Membership Coordinator, this form does not
need to be submitted. A Physical Examination Report is good for two years. If
you are not sure if your child’s form is current, check with Kathryn Devine at
(414) 258-3069 or devine61227@sbcglobal.net. YOUR CHILD MAY NOT
PARTICIPATE IN THE PLAYER EVALUATION, PRACTICES OR GAMES
WITHOUT A CURRENT PHYSICAL EXAMINATION REPORT ON FILE! NO
EXCEPTIONS! For volleyball, the physical examination form is due on or before
August 16, 2010.
After May 17th, completed registration packets can be returned to the Christ King
School Office. In addition, forms can be mailed or dropped off at Kathryn
Devine’s home at 2544 N. 83rd St, Wauwatosa.


Player Evaluations

 Children who are interested in playing volleyball for the ’10 season need to put
the following dates/times on their calendars. This particularly applies to 7th and
8th grade athletes who want to be considered for “A” team play.

SUMMER VACATION IS NOT CONSIDERED AN EXCUSED ABSENCE FROM
PLAYER EVALUATIONS FOR 7TH AND 8TH GRADE ATHLETES. IF YOUR
CHILD IS UNABLE TO ATTEND THE PLAYER EVALUATIONS, HE/SHE WILL
BE PLACED ON THE “B” TEAM! NO EXCEPTIONS!

IF YOUR CHILD QUITS AT ANY TIME AFTER THE BEGINNING OF THE
PLAYER EVALUATIONS, THE ATHLETIC COMMITTEE RESERVES THE
RIGHT TO ADMINISTER VARIOUS SANCTIONS AGAINST YOU AND/OR
YOUR CHILD INCLUDING BUT NOT LIMITED TO: NOT ALLOWING THE
CHILD TO PARTICIPATE IN OTHER SPORTS AT CHRIST KING, NOT
ALLOWING THE CHILD’S PARENTS TO COACH AT CHRIST KING,
IMPOSING A FINE ON THE PARENTS.

Player evaluations will take place for 5th grade athletes if there are enough
players to comprise two teams, both boys and girls. Teams are divided with
equal talent on both teams and teams remain intact for 6th grade. Players that
are new to 6th grade will be added to the teams that were formed in 5th grade.

Player evaluations will be added to the schedule for 6th grade boys, 7th grade
girls and 8th grade boys should athletes new to volleyball register this year.
These three teams will then be individually notified and evaluations would take
place on Wed, 8/18 and Thurs, 8/19.

7th and 8th grade player evaluations will take place to divide players onto an “A”
and “B” team. Players will be placed on teams according to the talent that is
demonstrated at the player evaluations. To be considered for “A” team play, a
player MUST be present at the player evaluation.
General Information

Practices: 5th and 6th grade teams have 2-75 minute practices each week and 7th
and 8th grade teams have 2-90 minute practices each week. Athletes will be
contacted by their coaches following the player evaluations and the coach will
advise the athlete of the days/times that they will practice.

Games: 5th-8th grade boys generally play league games on Friday evenings. 5th
and 6th grade girls generally play league games on Saturday mornings and 7th
and 8th grade girls generally play league games on Saturday afternoons. 5 th and
6th grade teams have the opportunity to play in two additional tournaments and
7th and 8th grade teams have the opportunity to play in three additional
tournaments. Individual coaches are responsible for securing tournament play
and registration.

Player/Parent Commitment: Players are responsible for attending all practices
and games. Parents are responsible for providing/arranging for their children to
and from all practices and games. The same level of commitment is expected of
all players, regardless of the team the child is assigned to. Please take the
commitment to play volleyball seriously and encourage your child to do so as
well.

Coaching: Volunteer parent/family coaches are vital to the success of our
volleyball program. If you are interested in coaching a 5th-8th grade girls or boys

Communication: My preferred method of communication is email. PLEASE
ensure that an accurate email address is provided on your Sign-Up Form if you
are interested in receiving general information as the volleyball season
progresses. Coaches will provide you with information that is specific to your
team.
