B) Executed authorization for credit report (attached).
C) One (1) months' complete pay stubs, retirement, social security or other benefit
D) One (1) year W-2s and/or 1099s plus complete Federal tax returns if you are self-
employed or claiming rental income or other income.
E) One (1) month recent bank and/or broker statements.
F) Copy of purchase contract (if buying a home).
G) Complete copy of divorce decree and/or separation agreements.
H) Transfer orders/papers if relocating.
I) Ratified lease agreements on any rental properties.
J) Name, address and phone number(s) of current and previous landlords for the last
two (2) years.
K) Sales contract(s) on home(s) sold within the last twelve months.
L) Copy of school transcript(s) or diploma if employed less than two years.
M) Copy of Deed of Trust and Survey for subject property (if refinancing). For VA &
FHA loans, Deed of Trust must show current agency number.
N) Copy of auto title(s) owned free & clear for vehicles less than five years old.

A) Original Certificate of Eligibility. If you do not have one we will provide you
with the forms necessary to obtain one.
B) Active Duty status letter or DD214.

To obtain a copy, I/we must send the lender a written request at the mailing address the lender has
provided. The lender must hear from me/us no later than 90 days after the lender notifies me/us about
