                               Washington, DC Chapter of Concerned Black Men, Inc.® {CBM-DC}.

                               This dual-purpose application must be used for both the Student of the Year as
                               well as the Scholarship Applicant. Feel free to photocopy the application as
                               needed. There is no limit to the number of students that can apply. Please post
                               the instructions where all students will have access to it.
