Please accept and return signed the following Conflict of Interest Disclosure Form (COI) before
continuing further with adjudication, review, panel discussion, decision making regarding Civil
No. CV 11-0057 HA in any way. I request that Judge Hernandez, Judge Haggerty or any other
Judge, Clerk, Investigator sign this Conflict of interest form to ensure there is no bias in any part
of the decision making proces of Civil No. CV 11-0057 HA, Oregon District Court.
It is not lawful to review, judge on, investigate any part of this case if there is a Conflict of
Interest between Panels of an kind, Investigators, court clerks, judges or anyone else that
decides the fate of defendant Crystal L. Cox on behalf of Plaintiff Kevin Padrick Obsidian
Finance or his attorney of record David Aman, Tonkon Torp, LLP.
Any deciding individuals in this matter in anyway must sign this form if there is any connection,
present, past or future with anyone at Tonkon Torp Law Firm or Obsidian Finance LLC. Or with
David Aman or Kevin Padrick personally or professionally in Anyway what so ever.
I request that any and all judges, clerks and those in any decision making roles related to United
States District Court, District of Oregon, Portland Division Civil Cas No. CV 1100057 HA sign
this form guaranteeing that there is No Conflict of Interest in this matter.
Failure to answer this conflict of interest letter may result in Criminal and Civil Charges against
you. This Conflict of Interest Disclosure Form is designed to ensure that the review and any
determinations from such review, judgement or investigation of the all materials involved in
this case will not be biased by any conflicting financial interest or any other conflicting interest
by those reviewers, judging, or deciding in any way, or responsible for the handling of this
confidential information in any way.
Whereby any conflict with the plaintiff or his attorney in any way must be fully disclosed and
affirmed in writing and returned for review prior to any action taken against defendant in any
way.

demanded not to open the remainder of the documents or open in any manner and instead
forward the matters on to the next available reviewer that is free of conflict that can sign and
complete the requisite disclosure.

Associations, Disciplinary Departments and any/all other appropriate oversight agencies for
failing to follow well established rules and regulations governing public office conflict, attorney
conduct conflicts, judicial conduct and law.
