OPEN ENROLLMENT/FLEX PLAN FY 2011-2012
TO: ALL EMPLOYEES
Open Enrollment for the insurance products covered under our Flex Plan will begin on
Tuesday September 27, 2011 and end on Thursday October 27, 2011.
Please be advised that if you are NOT MAKING A CHANGE to your coverage this year, you
DO NOT have to sign a new enrollment form. If your deductions are sheltered at this time,
they will remain sheltered effective January 1, 2012.
If you want to change the tax status of your deductions from either unsheltered or sheltered, or
make a change to the insurance products, please meet with a Colonial representative.
A PHOTO ID (School ID, Driver’s License, etc.) will be required for Flex Plan enrollment.
Please present ID to representative at the time of your enrollment.
If you are unable to meet with the enrollers on the scheduled date(s) assigned to your school or
department, or if your school or department is not assigned an enrollment date, you may enroll
at any location. The list of locations with the scheduled dates is attached for your convenience.
The open enrollment schedule can be found on payroll services web site at
http://payroll.jppss.k12.la.us.
