ALL pages of the signed Application & Agreement form.
A copy of your Federal 501c3 letter from the IRS. Note: If you cannot provide a copy
of your Federal 501c3 letter from the IRS, the Federal Trade Commission requires
LexisNexis to perform a brief site inspection of your office. Please read Attachment C for
more information.
You will be required to provide the product Internet Protocol (IP) address. An IP address is a
unique numeric label assigned to your computer. To find the IP address of your specific
computer, log on to the following website: http://support.lexisnexis.com/ip/report.asp. You may
disregard the section that asks for the IP Address Range and computer IP addresses for
administrator contacts.
The entire activation process may take up to seven business days once LexisNexis has received
a completed application. Plan for an additional three days, if a site visit is needed. Once the
activation process is finalized, the Account Administrator(s) will receive an e-mail from LexisNexis
with their User ID and log-in information. You’ll also be invited to attend a conference call
training, or 24/7 on-line training. Both options are a great way to become familiar with how to
place a background check order.
