Stuttering Rating Scale: 1= no stuttering
2= very mild stuttering for your child
10= very severe stuttering
Confidence Rating Scale: 1 = speaking freely
10 = silent to avoid stuttering
Usually, you will record an overall rating for the day. If, however, stuttering has varied a lot,
please record the range, for example 2-7 would indicate part of the day stuttering was very mild
and part of the day your child was stuttering quite a bit.
Comments: This can be anything you think is important or which you would like to have in your
notebook. You might record the activity you did during your treatment session, observations
about easier or more difficult situations, or descriptions of the stuttering events.
