"LAFAYETTE DISTRICT" - WEEKLY PROGRESS OR DRILL REPORT
This report is to be filed WEEKLY, by the operator, on all new wells from the time a Permit to Drill is issued until the well is either
Completed or Plugged and Abandoned, whichever comes first. When a work permit is issued to sidetrack an existing well (LST) or
to deepen an existing well (LDW) a report is required to be filled WEEKLY, by the operator, until the well is either Recompleted or
Plugged and Abandoned, whichever comes first. Show the present condition and all significant activities during the previous seven
days. A report may be called into the office (337) 262-5777 or Faxed to (225) 242-3328 or Emailed to (laf-drill@dnr.state.la.us)
