RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS
AND INDEMNITY AGREEMENT

PDW 2008, The Faculty of Pharmacy and Pharmaceutical Sciences, and The Governors of the University of Alberta
their officers, directors, agents, contractors, employees, volunteers, members and representatives (all hereafter
collectively referred to as “The Conference Organizers”, are not responsible for any injury, loss or damage of any
kind sustained by any person while participating in PDW 2008 (bus and driver provided by Golden Arrow) and all
related activities of the trip, including injury, loss or damage which might be caused by the negligence of the
Conference Organizers.
ASSUMPTION OF RISKS
In consideration of my participation in PDW 2008 and all related activities, I acknowledge that I am aware of the
possible RISKS, DANGERS AND HAZARDS associated with PDW 2008 including the possible risk of severe or
fatal injury to myself or others. These risks include but are not limited to:
a) the risks associated with travelling on a commercial bus line, which could include but are not limited
to a vehicle accident resulting in severe physical injuries or death;
b) intoxication and/or alcohol poisoning from the alcohol I consume whether voluntarily or through
coercion resulting in illness, injury or death;
c) the possibility of bodily injury (broken bones and soft tissue damage) including dental damages from
falling down, injuries incurred while getting on or off or moving about on the bus (in or out of the
mode of transportation) being used for the activities, being knocked down or being involved in a
physical confrontation whether caused by myself or someone else,
d) injuries or death from participating in any of the activities planned by the Conference Organizers or
any activities which I may choose to participate in separate from the planned activities of the
Conference while on the trip; and
e) the risks associated with returning to my residence once the conference has commenced.
I freely accept and fully assume all such risks, dangers and hazards and the possibility of personal
injury, death, permanent disability, property damage or loss resulting thereof.
INDEMNIFICATION AND RELEASE OF LIABILITY
In return for the Conference allowing me to voluntarily participate in PDW 2008 and all related activities, I agree:
1. TO ASSUME AND ACCEPT ALL RISKS arising out of, associated with or related to my participating
in PDW 2008 and all related activities, even though such risks may have been caused by the negligence of the
Conference organizers;
2. TO RELEASE THE CONFERENCE ORGANIZERS from any and all liability from any loss, damage,
injury or expense that I may suffer, or that my next of kin may suffer as a result of my participation in PDW
2008 and all related activities due to any cause whatsoever, including negligence, breach of contract, or breach
of any Statutory or Other Duty of Care, including any Duty of Care Owed under the Occupiers・Liability Act,
RSA 2000 C. 0-4 on the part of the Conference;
3. TO HOLD HARMLESS AND INDEMNIFY THE CONFERENCE ORGANIZERS:
a) from any and all liability for any damage to the personal property of, or personal injury to, any third party
resulting from my participation in PDW 2008 and all related activities;
b) for any cancellations, injury, loss, accident or delay occasioned by the proprietor, employee, or service of
any hotel, hostel or other type of accommodation used during PDW 2008 and all related activities.
c) from any and all claims, demands, actions and costs which might arise out of my participating in PDW
2008 and all related activities, even though such claims, demands, actions and costs may have been caused
by the negligence of the Conference organizers.

having the information in this document collected by the Conference Organizers. The personal information
requested on this form is collected under the authority of the Universities Act and Section 33 (c) of the FOIPP Act.
Certain personal information may be made available to federal and provincial government departments and agencies
under appropriate legislative authority. Personal information is protected under the Alberta FOIPP Act. For further

ACKNOWLEDGEMENT
I acknowledge that I have read this agreement, that I have executed this agreement voluntarily, and that this
agreement is to be binding upon myself, my heirs, executors, administrators and representatives, in the event of my
death or incapacity
