In recognition of Rotary International’s 106th anniversary and our 26-year fight to eliminate polio
from the world, we request the honor of projecting the Rotary logo and our End Polio Now message
onto the (INSERT BUILDING OR STRUCTURE) on February 23, 2011.

When Rotary began its mission to eradicate polio in 1985, more than 350,000 children were stricken
by the crippling disease every year. Since then, the number of polio cases has been reduced by 99
percent, to fewer than 1,700 cases in 2009. Today, only four countries – Afghanistan, India, Nigeria,
and Pakistan – remain polio-endemic. Owing to the efforts of Rotary and its partners, the world is on
the threshold of eliminating polio, making it only the second disease (after smallpox) to be eradicated.

To date, Rotary has contributed more than $900 million and countless volunteer hours to immunize
more than two billion children – and our work continues. Rotary clubs worldwide are in the midst of a
campaign to raise an additional $200 million toward $355 million in challenge grants from the Bill &
Melinda Gates Foundation. These funds will provide critical support to polio eradication activities.
We are nearing completion of this goal, having raised over $140 million.

In our ongoing efforts to raise awareness of polio eradication efforts, we have coordinated with the
managers of other iconic structures worldwide to spread our message. In the past two years, we have
projected the End Polio Now message on the Sydney Opera House, the British Houses of Parliament,
the Roman Colosseum, the Great Pyramid of Khafre near Cairo, the Wrigley Building in Chicago, and
several castles in Scotland. We believe that a lighting projection on the (INSERT BUILDING), one of
the most prominent buildings in (INSERT CITY), will help increase awareness of our fight to end
polio now.

With more than (INSERT NUMBER) Rotary members in the (INSERT CITY) area and 1.2 million
members worldwide, we are confident that there would be significant interest in this event,
particularly given the determined effort of Rotary volunteers to raise the necessary funds in this last,
most difficult stage of the eradication initiative.
