previously evaluated courses. The Evaluation Data System (EDS) can be found at:
http://gazos.ucsc.edu/eds/main.cfm. Prior to submitting this form, you may want to visit that
web site to see if the transfer course information you seek has previously been evaluated.

Admissions does not review courses taken from the following: California community colleges, UC, UC
Extension, and UCSC Concurrent Enrollment through UC Extension. You are limited to no more than 5
courses for review from a single institution. We will review no more than two institutions for a maximum of
8 courses.

institution sent directly to the Office of the Registrar. Application of this credit toward major requirements
is subject to the approval of the appropriate department.

week devoted to lecture and lab, and a brief description of the course.
