SAMPLE – Student Data Collection Form .................. ........................................................23 Purpose This Preparation Manual for Testing and Reporting provides the information and resources needed by district physical fitness test (PFT) coordinators to prepare for and carry out the spring 2009 administration of the PFT. This document provides background information and directions for administering the PFT as well as information related to the collection and reporting of test results to the state PFT contractor, Educational Data Systems (EDS 1). PFT coordinators are strongly encouraged to read the entire manual, paying close attention to the requirements for compiling and reporting the PFT results. Further information about the PFT can be obtained by accessing the Web sites and contacts provided throughout this document. 1 EDS is a registered trademark of Hewlett-Packard Development Company, LP. Administering the Physical Fitness Test Student Participation All students in grades five, seven, and nine must be administered the PFT during February 1, 2009 through May 31, 2009. The information in this manual applies to the PFT for students in these three grades only. It does not apply to students who may be tested in grades ten or higher to determine exemption eligibility. Students with Disabilities Students with disabilities who are unable to take the entire PFT should be given as much of the test as conditions permit. The individualized education program (IEP) or Section 504 plan team is responsible for deciding how children with disabilities will participate in the PFT. Matrix 1. Matrix of Test Variations, Accommodations, and Modifications for Administration of California Statewide Assessments provides a list of the types of variations and accommodations available for the PFT. This matrix is posted on the California Department of Education (CDE) Student Testing Web page at http://www.cde.ca.gov/ta/tg/sa/. Test results should be reported for all grades five, seven, and nine students with disabilities who participate in the PFT. The results should reflect the extent to which the students were able to participate in any part of the test. Students on a Block Schedule The requirement to administer the PFT includes students who attend schools that are using a block schedule and whose students may not be enrolled in physical education classes during the PFT administration window (i.e., February 1 through May 31). More importantly, if schools in the school district are on a block, quarter, or alternative schedule in which students do not receive physical education instruction every 10 school days, these schools are in violation of the California Education Code sections 51210 and 51222. The State Board of Education (SBE) may grant waivers of the requirement for physical education instruction every 10 school days for schools that meet the specific criteria established in SBE Policy #99-03. One of the criteria requires that “Students are prepared for and participate in the physical performance testing as specified in the California Education Code.” A copy of the Physical Education Block Schedule Waiver Criteria can be obtained from the CDE Waivers Web page at http://www.cde.ca.gov/re/lr/wr/. Test Administration, Materials, and Training The FITNESSGRAM , developed by The Cooper Institute and distributed through Human Kinetics, continues to be the required PFT designated by the SBE. The district PFT coordinator should determine the needed test materials for each school and place orders as early as possible. Test Administration Manual The fourth edition of the FITNESSGRAM Test Administration Manual is designed to make the ® FITNESSGRAM easier to understand and use in physical education classes. The manual describes in detail how each test is administered, what is measured, and what limits are indicated for the tests (e.g., scores on the Trunk Lift are limited to 12 inches to discourage hyperextension). Both the third and fourth editions of the manual also include an instructional DVD with video clips to help teachers ® properly administer the FITNESSGRAM . The fourth edition manual includes a CD for the 15-meter and 20-meter PACER (Progressive Aerobic Cardiovascular Endurance Run), as well as cadences for ® ® the Curl-Up and Push-Up tests. (Cadences help students pace their movements during these tests.) Furthermore, due to the addition of the 15-meter PACER, the fourth edition manual includes a conversion table for converting the 15-meter laps to 20-meter laps, which must be done for PFT recording and reporting purposes. This table can also be viewed and downloaded from the ® FITNESSGRAM Web site at http://www.fitnessgram.net (Outside Source). Previous editions of the ® FITNESSGRAM Test Administration Manual may continue to be used for test administration. FITNESSGRAM Trainings ® To better understand the philosophy and requirements of the FITNESSGRAM , district PFT ® coordinators may consider participating in face-to-face FITNESSGRAM trainings, which may be available locally, or by completing the free online training course offered by Human Kinetics. District ® PFT coordinators and teachers can access this online training on the Human Kinetics FITNESSGRAM Training for the Test Protocols and Software Web site at http://www.fitnessgram.net/training (Outside Source). This online training is not designed to comply with Education Code PFT requirements. For example, the PFT must be administered by an employee of a school district and not a student, as shown in the online training. In addition, videos and training materials from the CDE PFT training sessions for district PFT coordinators are available on the Internet at http://www.pft-info.org/ (Outside Source). Options for Measuring Body Composition The two tests for measuring body composition discussed in the FITNESSGRAM Test Administration Manual are Skinfold Measurements and the calculation of Body Mass Index. In addition to these two methods, automated skinfold calipers and bioelectric impedance analyzers (BIAs) are permitted for measurement of body composition (i.e., percent body fat). In recent years, there has been an increase in the use of BIAs for measuring body composition. A number of new portable devices are now available, which are inexpensive, easy to use, and produce accurate results. Correlations of body compositions indexes obtained with skinfold measurements and BIAs were found to be high for all comparisons (exceeding .80 for both boys and girls), indicating there is good overall agreement between the measurement approaches. One device resembles a bathroom scale and produces an estimate of body composition when the participant stands on the device (in bare feet). Another handheld device estimates body composition when the participant grasps its two handles. The Cooper Institute recommends the Omron handheld device since it yielded the most accurate comparisons with skinfold measurements. It is important to note that a student’s hydration status and prior exercise can influence accuracy of individual estimates from BIAs; therefore, manufacturer guidelines should be closely followed when using these devices. While the BIAs described above have been shown to be reasonably accurate, those based on the infrared interactance method (e.g., Futrex) have not been shown to provide accurate estimates of body composition in children. Therefore, BIAs based on infrared interactance methodology cannot be used as part of the PFT. Ordering Materials FITNESSGRAM materials are not available from the CDE. The entire FITNESSGRAM program, which includes software, the manual, and other administration materials, may be purchased separately from Human Kinetics. Scannable forms (used for recording scores) and scoring and reporting services are available from fee-for-service vendors. Additional information about the FITNESSGRAM may be found on the FITNESSGRAM Web site at http://www.fitnessgram.net (Outside Source). ® ® ® ® ® ® Note: The FITNESSGRAM software data export feature does not conform to California PFT data collection and reporting requirements. Special instructions to export PFT data for ® FITNESSGRAM software users are available on the EDS California Physical Fitness Test 2009 District Portal Web site at http://www.eddataonline.com/fitness/ (Outside Source). Human Kinetics should be contacted for information about FITNESSGRAM materials only. They are not prepared to respond to questions about PFT administration policies, data collection, or electronic data transmittal. If you have further questions regarding PFT administration policies, please contact the California PFT coordinator at 916-445-9449 or e-mail at PFT@cde.ca.gov. ® ® Collecting and Reporting the Results The PFT results are collected and reported every year. State, county, school district, and school results are posted on the Internet at http://data1.cde.ca.gov/dataquest/. The California Department of Education (CDE) also submits a report of the state PFT results to the Governor and the Legislature. The completion of these tasks requires that school districts submit their PFT results to the state. The following table outlines the activities and schedule for collecting and reporting the PFT data. Activity Student Testing School districts submit PFT results to the CDE (via the state PFT contractor, EDS) PFT results available on the Internet Report of results submitted to the Governor and the Legislature Date February 1 to May 31 No later than June 30 No later than December 1 No later than December 31 It is the responsibility of the school district to determine how student test data will be collected and recorded. The school district or the fee-for-service vendor chosen by the school district to report its data is responsible for compiling student data in a required electronic format to send to the state PFT contractor. Currently, the state PFT contractor is Educational Data Systems (EDS). Collecting School Results and Compiling the Data “Collecting and Reporting Data” and “Directions for Electronic Reporting Options” (Appendixes A and B, respectively), provide a variety of steps a school district could take to collect student data and transfer it to an electronic format. These suggestions, while not exhaustive, represent a summary of the procedures school districts have used in past years. Submission of data files via e-mail is not an option. Data to Be Collected and Reported PFT data to be collected and reported by school districts must include two files 1) a school record file (Appendix C) with one record for each school and 2) a student data file (Appendix D) with results for every student in grades five, seven, and nine, whether or not the student was fully tested, partially tested, or not tested at all. This includes data for students with disabilities who participated in any part of the PFT. However, data for any students tested in grade ten or higher to determine exemption eligibility should not be included in data submitted to EDS. Note: Data will not be accepted without completed forms for all schools and students. Contact information, including a name, phone number and e-mail address, is also required in case there is a question regarding your submission. Data submitted without a school record file will be considered incomplete. Special care should be taken to make sure that all students have their date of birth and gender recorded correctly. Without this information, student results cannot be calculated. Data Collecting and Reporting Services The information to be collected can be found on the school and student record layouts (Appendixes C and D) and the sample school and student data collection forms found in Appendixes F and G. Information is the same on the respective forms, but the sequence of the fields differs. If a school district plans to report school and student data using a CD-ROM, diskette, or via Web upload, the record layouts (Appendixes C and D) should be used. Because there have been some changes to some of the specifications, please carefully review “Appendix C – School Record Layout” and “Appendix D – Student Record Layout.” If data will be entered or uploaded online, the sample collection forms may be used to record the data prior to their entry or upload. School and student data collection forms (samples provided in Appendixes F and G) should not be sent to EDS. Cost Reimbursement All costs incurred to administer, score, analyze, and report results of the PFT are recoverable as “state-mandated costs.” The Office of the State Controller administers cost recovery. This office may be contacted at 916-324-5729. SAMPLE – Student Data Collection Form Appendix A 2008–09 Physical Fitness Test Collecting and Reporting Data Suggestions for School Districts The following suggestions for collecting student test results provide methods that have been used by school districts in past years. School districts may have other procedures they wish to use. In this table, the current state PFT contractor refers to Educational Data Systems (EDS). Appendix B 2008–09 Physical Fitness Test Directions for Electronic Reporting Options Reporting Physical Fitness Test (PFT) Data: Once the data from the PFT has been collected for each school, school districts or their vendors have the following options for reporting the results to Educational Data Systems (EDS), the state PFT contractor: 1. Web site data entry: This is a good option for school districts with small numbers of students. The school district or school staff enters individual student data one-by-one into a Web site datacollection form. To select this option, you need the district PFT coordinator’s password. To obtain the password, see instructions in the “Obtaining a District PFT Coordinator’s Password” section below. 2. Web site data upload: Data which is in plain text ASCII with fixed width or uses the EDS Excel or Access data entry templates may be submitted online via a special upload form. To select this option, you need the district PFT coordinator’s password. To obtain the password, see instructions in the “Obtaining a District PFT Coordinator’s Password” section below. 3. CD-ROM or diskette in standard format files: Once individual student data and school demographic data are compiled, this data is downloaded onto a CD-ROM or diskette and mailed to EDS via a traceable shipping method (not e-mail) to EDS. Note: Submission of data files via e-mail attachments is not an option. The EDS California PFT 2009 District Portal Web site at http://www.eddataonline.com/fitness/ (Outside Source) is provided for collecting student data from each school district. This site also includes information on file layouts and data submission options. Many features on this secure site require a password. Obtaining a District PFT Coordinator’s Password: A district password will be e-mailed to the designated district PFT coordinator. Each school district and charter school must designate a district PFT coordinator who will be contacted by EDS or the CDE about updates to the PFT, timeline requirements, or problems with the data. Once completed, a contact survey form needs to be signed by the district superintendent and faxed to EDS at 408776-7696. The contact form can be downloaded from the California PFT 2009 District Portal Web site at http://www.eddataonline.com/fitness/ (Outside Source). Beginning in 2008-09, a new contact information form must be submitted by November 1 each year, even if the information has not changed from the previous year. Contact information forms received on or after August 1, 2008, will be considered as submitted for the 2008-09 school year. Appendix B (Continued) 2008–09 Physical Fitness Test Directions for Electronic Reporting Options Standard File Formats: EDS will accept data in a number of standard Windows PC file formats, as listed below. 1. ASCII fixed-width text file: Files in this format are relatively small. Please read the school and student record layouts for details on the file specifications required for this format (Appendixes C and D). 2. MS Excel/MS Access Templates: These files are larger than the ASCII files, and Excel or Access must be installed on your computer. You may download a pre-formatted template (in two different formats) from the EDS Web site or you may contact EDS for assistance. This option includes Excel 97/2000 or Access 2000. Note that the Excel template includes two worksheets and a Help page. Only one file from each school district will be accepted. This file must contain records for all students within your district who are enrolled in grades five, seven, and nine. 3. FITNESSGRAM Software Export: Districts which use the FITNESSGRAM software to collect ® the student fitness data need to know that the FITNESSGRAM software export feature does not conform to California PFT data collection and reporting requirements. Some fields from the ® FITNESSGRAM export must be deleted and additional fields are required for PFT data ® submission. In addition, the FITNESSGRAM software export feature does not create the school record file. This file must be entered manually using the ASCII fixed-width text file or the MS Excel/MS Access templates. Special instructions that must be followed to export PFT data for ® FITNESSGRAM software users are available on the EDS California PFT 2009 District Portal Web site at http://www.eddataonline.com/fitness/ (Outside Source). Macintosh Users: Macintosh users may use the same data submission methods and file formats described above. Since files created on a Macintosh look different on a Windows PC, please contact EDS to inform them that a Macintosh was used to create the data files. Formatting Data: Please leave entries blank or null for tests not taken by a student. The scoring programs treat a result of zero differently from a blank. A zero indicates a test taken with a resulting score of zero, such as 0 push-ups were achieved. A blank indicates a test was not taken. EDS’ primary function is to collect the annual PFT data from the school districts and to report these results to the California Department of Education (CDE). These results are subsequently posted to the CDE DataQuest Web page at http://data1.cde.ca.gov/dataquest/. It is the responsibility of the school district to determine how student PFT data will be collected, recorded, and reported to EDS. Please read the following answers to frequently asked questions prior to contacting EDS with questions. 1. What is required to be submitted to EDS? The following two files are required from every district: • A student data file with the results identified in the student file layout. • A school summary data file, which contains sums of the student data for each school and grade. If using the EDS California PFT 2009 District Portal Web site at http://www.eddataonline.com/fitness/ (Outside Source), the data entry screens prompt the user to enter the correct data for each student and grade. 2. What file layout should be used? Depending on local preferences and how the student data has been collected, one of the following three approved student file layouts should be used: • ASCII fixed-width text file • • Excel spreadsheet template Access database template If opting for the Excel or Access templates, the actual templates must be used and not a replica of the template built by the district. The templates are available from the EDS California PFT 2009 District Portal Web site at http://www.eddataonline.com/fitness/ Outside Source). 3. In the Excel spreadsheet template, there are multiple worksheet tabs at the bottom. Which one(s) should be completed? There are three worksheet tabs in the Excel template: • Tab One is a help screen only. Users should not enter data into the help screen. • • Tab Two is for entering the summary data for each school and grade. Tab Three is for entering the student data, one row for each student, whether they were fully tested, partially tested, or not tested at all. Should a school district submit a data file exported from the (optional) FITNESSGRAM software? ® ® No. For the 2008-09 school year, all FITNESSGRAM software users must comply with the CDE ® requirements and send data in one of the approved file layouts. Since the default FITNESSGRAM export file does not comply with all the format and layout requirements, please refer to the special ® instructions for creating usable data files exported from the FITNESSGRAM software. Special ® instructions for FITNESSGRAM software users are available on the EDS California PFT 2009 District Portal Web site at http://www.eddataonline.com/fitness/ (Outside Source). 5. What are the accepted methods of transferring the district’s PFT data to EDS by June 30? The acceptable methods include data entry through the secure EDS Web site, secure electronic file upload through the EDS Web site, and on a CD-ROM (mailed to EDS via a traceable shipping method). The EDS California PFT 2009 District Portal Web site can be accessed at http://www.eddataonline.com/fitness/ (Outside Source). 6. Can PFT data be e-mailed to EDS? E-mail is not a secure form of data transmittal and, therefore, is not an available option for sending data. 7. What is the mailing address to which districts must send their data files on CD-ROM? Educational Data Systems Attn: Physical Fitness Coordinator 15850 Concord Circle, Suite A Morgan Hill, CA 95037 408-776-7646 Please mail using a method that has a tracking number. 8. Are all students in grades five, seven, and nine required to take the PFT? Yes. California requires all students in grades five, seven, and nine to take the PFT. 9. What data is entered for each student who was not tested? Some students have a valid reason for not testing. The following are the valid reasons and the codes for students not tested: • 10 – Absent on test date and all make-up sessions for all tests • • • 13 – Individualized education program (IEP)/Section 504 plan/Disabilities 14 – Extraordinary circumstances 15 – Medical excuse The district must provide the demographic information and a code for the reason not tested for every student who did not test. Appendix E (Continued) 2008–09 Physical Fitness Test Frequently Asked Questions 10. What data is entered for each student who was partially tested? For each student who was partially tested, districts must provide all of the student data including the student’s demographics and the test results for each of the tests he/she took. Leave the fields blank for tests that were not attempted (i.e., do not fill in these fields in with zeros). Additionally, districts must provide a valid reason and a code for incomplete data as follows: • 20 – Absent on test date and all make-up sessions for one or more tests • • • 23 – Individualized education program (IEP)/Section 504 plan/Disabilities 24 – Extraordinary circumstances 25 – Medical excuse 11. What demographic information is required for each student? The demographic fields and their acceptable values and ranges are provided in Appendixes D and G. The ethnic codes provided by the district must be one of the California-approved 3-digit codes. If applicable, the reason for not testing must be one of the listed 2-digit codes (see Question 9). Students who did not take all six tests must also have a reason for partial testing using one of the 2digit codes for partially tested (see Question 10). Beginning with the 2008-09 school year, the PFT data must now include a Statewide Student Identifier (SSID) number for each student. 12. Is the SSID the same as the CSIS number? Yes. All local educational agencies are required to assign and maintain California School Information Services (CSIS) Statewide Student Identifier (SSID) numbers for all students. More information on SSID numbers is available from the CSIS Statewide Student Identifier Web site at http://www.csis.k12.ca.us/library/statewide-identifier/default.asp (Outside Source). 13. Does the EDS Web site data-collection form include a space for entering the SSID numbers? Yes. The EDS Web site data-collection form contains all of the fields provided in the student file layout including SSID numbers and first day of testing. Furthermore, the Web site data-collection form only accepts score values that are within the allowable ranges (e.g., no scores higher than 12 inches for the Trunk Lift). 14. What are some common data entry errors made by districts? Aerobic Capacity • The Mile Run results are often filled in under the Walk Test. • In addition to the minutes and seconds, the Walk Test also requires the 15-second heart rate and the student’s weight. If these data are not included, the Walk Test score cannot be calculated. The Walk Test is given to students younger than age 13, for which there are no performance standards provided. It should only be administered to students ages 13 or older. • Appendix E (Continued) 2008–09 Physical Fitness Test Frequently Asked Questions Body Composition • Both the height and weight are needed to calculate the Body Mass Index (BMI). • When entering the height for BMI, convert 12 inches to the next higher foot (e.g., 4'12" should be entered as 5 in the feet field and 00 in the inches field). Both the Abdominal Strength and Trunk Strength tests are required. School districts often only administer one of these tests. Abdominal Strength has a maximum of 75 curl-ups, therefore, values over 75 are not allowed. Trunk Strength (Trunk Lift), has a maximum of 12 inches, therefore, values over 12 inches are not allowed. The two Flexibility tests require measurements on both the left and right side. Without data for both sides, the Flexibility score cannot be calculated. The Back-Saver Sit and Reach test has a maximum of 12 inches, therefore, values over 12 inches are not allowed. Other Tests • • • • 15. When does one record a 0 for a score or leave the score blank? A score of 0 indicates the student attempted a test and scored a 0 (e.g., 0 inches, or 0 push-ups). For the Mile Run and Walk Test a score of 99 minutes and 59 seconds indicates that the student attempted the mile run or walk test but did not complete the test. A blank indicates that the test was not administered to the student. Any student records with blanks should include a valid reason the student was not tested or partially tested (see Questions 9 and 10). 16. What is required if a student attempts the Mile Run or Walk Test but does not finish? If a student cannot complete the distance, fill in 99 minutes and 59 seconds for the time. 17. Why was the date used for determining the student’s age changed from April 1 to the first day of testing? In order to determine if a student’s score falls in the Healthy Fitness Zone (HFZ) for a given test, the student’s score is compared to standards established by gender and age. In the past, the age of the student was determined as of April 1 (which is the mid point of the PFT testing window). Due to recent legislation, which raised the stakes of the PFT results for high school students, changing the date from April 1 to the first day of testing provides for greater accuracy in calculating age and evaluating student performance against the HFZ. Appendix E (Continued) 2008–09 Physical Fitness Test Frequently Asked Questions 18. What does EDS do with files submitted by a school district or a vendor (on behalf of a school district), which include scores that do not fall in the valid ranges? If EDS receives a file from a school district or vendor, which includes scores that do not fall in the valid ranges or have any other data problems [e.g., missing Statewide Student Identifier (SSID) numbers], school districts will be immediately contacted by EDS. While EDS works closely with school districts to ensure the files submitted contain data that is accurate and complete, school districts are responsible for the integrity of the data submitted. 19. The student’s age is limited on the data entry Web site. What if a student is older than the maximum age allowed? This situation is handled on a case-by-case basis. Enter the earliest year offered in the drop-down screen. Next, call EDS to modify the date after it’s been submitted. EDS will provide documentation that the birth date has been corrected in the data file. 20. How does a school district get individual student PFT results? Individual student PFT results are confidential and are not available from the CDE or the DataQuest Web page. The district PFT coordinator may access the individual student PFT results from the secure, password-protected EDS Web site, from a vendor (if the school district used outside vendor services), or from the student data file created by the school district to report individual student results to EDS. 21.
