As per the your latest letter as attached, and as per my telephone conversation with Debra
Badge # 2809319 on Dec. 2, 2004, I am sending a notarized copy of my passport which
Debra told me is the only document that was required to complete my application for an
ITIN number.
I have also enclosed a copy of a letter that I sent you on Oct. 19, 2004.
