MEDICATIONS
Permission to Administer Medications at School forms must be completed by parents and
health care provider for ANY medication to be given while at school. This includes Non-
prescriptive medications commonly referred to over-the-counter medications.
Examples are Tylenol, Advil, Ibuprofen, TUMS, Motrin, Homeopathic or “natural”
remedies. All Prescription medications require a completed form.
If parents/guardians sign the parent portion authorizing the health care provider to return
fax the form to us, we can fax the Permission to Administer Medication form to the
health care providers office. You would not need to make a separate trip to their office.
The completed forms are valid for one school year.
Students with asthma may carry inhalers with them with signed permission of the
parent/guardian, physician and agreement of the Einstein principal and nurse. The
student must follow the written procedures on page 4 of the Shoreline School District
Policy Manual #3416P. I encourage Middle School students to have their inhalers with
them, it is often a long way to the health office! Many students are turning out for sports
and other activities after school when the health office may be closed. I am happy (really
like) to keep an extra inhaler in the health office in case the one that is suppose to be in
their backpack gets left at home!
Students often take medications at home but is not required to take while at school. In an
event that keeps students overnight at school, it is recommended to have a three day
supply of medication at school for “emergency situations”. This is especially important
for students taking medication for:
ADD/ADHD
Depression/Anxiety
Asthma
Seizures
Diabetes.
