THIS AGREEMENT TO PROTECT CPNI (“CPNI Protection Agreement” or “Agreement”) is made on this 28th day of January,
2008 (“Effective Date”), by and among Verizon Business Network Services Inc., on behalf of MCI Communications Services, Inc.,
d/b/a Verizon Business Services and any applicable Verizon affiliates providing service to the Government (“Verizon”) and
Department of Networx, Test Bureau (“Customer”), through its authorized representative‟s signature above and Verizon‟s
acceptance of it. Verizon acceptance occurs upon Verizon's verification that an unaltered Customer-signed document is received
by a Verizon implementation center.
WHEREAS, Verizon is a contractor to the Networx Universal (GS00T07NSD0008) and Networx Enterprise (GS00T07NSD0038)
agreements with the General Services Administration (“GSA”), collectively referred to herein as the “Service Agreements”;
WHEREAS, the parties desire to establish terms under which Customer Proprietary Network Information (“CPNI”) may be
provided by Verizon to Authorized Customer Representatives (as defined below);
NOW, THEREFORE, in consideration of the mutual promises set forth below, and intending to be legally bound hereby, the
parties agree as follows.
Protection of Customer CPNI
a) Verizon will protect the confidentiality of Customer CPNI in accordance with the Service Agreements and applicable
laws, rules and regulations. Verizon may access, use, and disclose Customer CPNI as permitted or required by the
Service Agreements and such applicable laws, rules, and regulations or this Agreement.
b) Verizon may provide Customer CPNI (including, without restriction, call detail) to representatives authorized by
Customer (“Authorized Customer Representatives” as defined below) in accordance with the following:
Verizon may provide Customer CPNI to Authorized Customer Representatives via any means authorized by
Verizon that is not prohibited by applicable laws, rules, or regulations, including, without restriction: to the
Customer‟s e-mail address(es) of record (if any) or other e-mail addresses furnished by Authorized Customer
Representatives, to the Customer‟s telephone number(s) of record or other telephone numbers provided by
Authorized Customer Representatives, to the Customer„s postal (U.S. Mail) address(es) of record or to other postal
addresses furnished by Authorized Customer Representatives, or via Verizon‟s on-line customer portal or other on-
line communication mechanism.
c) “Authorized Customer Representatives” include those Customer employees, agents, or contractors, other than Verizon,
listed in “Authorization or De-authorization of Access to Customer Proprietary Network Information (CPNI)” forms that
are provided to Verizon by a Customer CPNI Authorizer (see below). Authorized Customer Representatives shall
remain such until a Customer CPNI Authorizer notifies Verizon in writing that an individual no longer is an Authorized
Customer Representative. Customer agrees, and will cause Authorized Customer Representatives, to abide by
reasonable authentication and password procedures developed by Verizon in connection with disclosure of Customer
CPNI to Authorized Customer Representatives.

d) Customer‟s “Authorization or De-authorization of Access to Customer Proprietary Network Information (CPNI)” forms
must be sent to the Networx User Administration Group at NXU-Registration@verizon.com and must contain the
following information:
- the name, postal address, and e-mail address of the person authorized (or deauthorized)
- the Agency Hierarchy Code(s) for which the person is authorized (or deauthorized) to access CPNI
- the services for which the which the person is authorized (or deauthorized) to access CPNI, if applicable
- that the person is being authorized (or is no longer authorized) to access CPNI
- the name of the Agency whose CPNI the person can access (or can no longer access)
e) During the period of performance of the Service Agreements, Customer will at all times have designated, below or in a
separate writing sent to the Networx User Administration Group, up to three representatives (“CPNI Authorizers”) with
the power to authorize Customer representatives to access CPNI. Additions or removals of CPNI Authorizers will be
effective within a reasonable period after Verizon has received a signed writing of the change, including the affected
person(s)‟ name, title, postal address, e-mail address, and telephone number.
