U.S. and Idaho

The number of Idaho resident deaths increased 1.7 percent from 10,742 deaths in 2007 to

10,927 deaths in 2008. The death rate remained constant at 7.2 per 1,000 for Idaho residents,

matching that rate from 2006 and 2007. There were 2,426,264 deaths in the United States in 2006

(latest year available), which was 21,753 fewer deaths than 2,448,017 in 2005. The 2006 U.S.

crude death rate was 8.1 deaths per 1,000 population compared with 8.3 in 20051.



U.S. and Idaho Cause of Death Ranking

The ten leading causes of death in U.S in 2006 were the same as the leading causes in

Idaho in 2008, with the exception of one cause: Septicemia. However the ranking of the leading

causes differed between the U.S. and Idaho. The first two leading causes in the U.S., Diseases of

heart (ranked first) and Malignant neoplasms (ranked second), were the second and first leading

causes, respectively, in Idaho. In the U.S., Cerebrovascular diseases ranked third, but ranked fifth

in Idaho. Chronic lower respiratory diseases (ranked fourth in U.S.) and Accidents (rank fifth in

U.S.) were ranked third and fourth in Idaho. In 2006, Diabetes was the sixth leading cause in the

U.S. (ranked seventh in Idaho), followed by Alzheimer’s disease (ranked sixth in Idaho), and

Influenza and pneumonia (rank ninth in Idaho). The ninth and tenth ranked causes in the U.S. were

Nephritis, nephrotic syndrome and nephrosis and Septicemia, which were ranked tenth and

fourteenth, respectively in Idaho. Intentional self-harm (suicide) ranked eleventh in the U.S. in 2006

and eighth in Idaho in 2008.

U.S. mortality rates are consistently much higher than those in Idaho for Diseases of the

heart, Malignant neoplasms, Nephritis, nephrotic syndrome and nephrosis, and Septicemia. Higher

Idaho rates are recorded for Intentional self-harm (suicide) and Accidents (unintentional injury).



Changes in Cause of Death Ranking in Idaho from 2007 to 2008:



Malignant neoplasms (cancer) became the leading cause of death, surpassing Diseases of heart

by 173 deaths. Diseases of heart became the second leading cause among Idahoans.



Cerebrovascular diseases (ranked fourth in 2007) and Accidents (ranked fifth in 2007) traded

rankings in 2008.

Intentional self-harm (suicide) (ranked ninth in 2007) and Influenza and pneumonia (ranked eight

in 2007) switched rankings in 2008.



Nephritis, nephrotic syndrome and nephrosis moved up to tenth rank in 2008 from eleventh in

2007.



Chronic liver disease and cirrhosis moved down to eleventh rank in 2008 from tenth rank in 2007.



Changes in Idaho Age-Adjusted Rates:

There were no significant2 changes in the age-adjusted rates for the ten leading causes between

2007 and 2008. However, of the ten leading causes, age-adjusted rates per 100,000 population

were higher in 2008 than in 2007 for:

Deaths to Idaho Males and Females

The leading cause of death for both males and females in 2008 was Malignant neoplasms

(cancer). Age-adjusted rates for males were significantly higher2 than age-adjusted rates for

females for six of the ten leading causes of death among males:

There was no significant difference in the age-adjusted rates between males and females for

Cerebrovascular diseases, Diabetes mellitus, Alzheimer’s disease, Influenza and pneumonia,

Nephritis, nephrotic syndrome and nephrosis, or Essential hypertension and hypertensive renal

disease. The age of the oldest male decedent was 104 and was 107 for the oldest female.

INFANT, FETAL, AND MATERNAL MORTALITY



U.S.

The U.S. infant mortality rate decreased from 6.9 in 2005 to 6.7 in 20061 (latest year final

data are available). The 2007 preliminary rate was 6.8 per 1,000 live births. 2007 preliminary data

indicate the leading cause of infant death in the U.S. was Congenital malformations, deformations

and chromosomal abnormalities (birth defects) (133.6 deaths per 100,000 live births). The second

leading cause in 2007 was Disorders related to short gestation and low birth weight (108.4 deaths

per 100,000 live births), followed by Sudden infant death syndrome (SIDS) (49.1 deaths per

100,000 live births)2. In 2007, there were 29,241 infant deaths in the U.S. (preliminary), compared

with 28,527 in 2006 and 28,440 deaths in 2005.



Idaho

In the past ten years, Idaho’s infant mortality rate fluctuated between record low rate in 2008

(5.8 per 1,000 live births) and a high rate of 7.5 in 2000. In 2008, there were 146 infant deaths,

which was a 13.6 percent decrease from the number of infant deaths in 2007 (164 deaths). The

infant mortality rate decrease 14.7 percent from 6.8 per 1,000 live births in 2007. Considering the

rates are based on small numbers, some irregularity is not unusual. The graph on page 102

illustrates the trend of Idaho's infant death rates as a one- to two-year "up-and-down" cycle.



Neonatal and postneonatal death rates also produce yearly changes in Idaho. The neonatal

mortality rate (infant deaths less than 28 days) in 2008 decreased to 3.9 from 4.5 in 2007; the

postneonatal rate (infant deaths from 28 days through 11 months) decreased to 1.9 from 2.2 per

1,000 live births in 2007.



District 5 experienced the highest infant mortality rate in 2008 at 7.1 per 1,000 live births

followed by District 3 (6.6). District 7 had the lowest rate in 2008 (5.1). Neonatal mortality ranged

from a low of 3.2 deaths per 1,000 births in Districts 2 to a high of 4.8 deaths per 1,000 births in

both District 3 and District 5. Postneonatal mortality ranged from a low of 1.5 deaths per 1,000 live

births in District 1 to a high of 2.6 in District 6.



Because Idaho infant deaths provide a small data base that tends to fluctuate or produce

abnormally high rates in small counties, moving three-year rates have been calculated. See page

107 for three-year infant mortality rates by district and county, from 1997-1999 through 2006-2008;

the overall trend is more readily apparent.



Leading Causes of Infant Death

In 2008, there were 69 Idaho resident infant deaths for the general infant death category,

"Certain conditions originating in the perinatal period" compared with 77 deaths in 2007. This

category includes such conditions as short gestation and low birth weight, respiratory distress of

newborn, and neonatal hemorrhage. In ranking the leading causes of infant deaths, however, the

conditions within this general category are used to determine leading causes of death, rather than

the general category. The leading cause of Idaho infant deaths was Congenital malformations,

deformations and chromosomal abnormalities (birth defects) (33 deaths; 131.2 deaths per 100,000

births). Sudden infant death syndrome (SIDS) and Disorders related to short gestation and low

birth weight were tied for the second leading cause of infant death (21 deaths each; 83.5 deaths per

100,000 births). Historically, SIDS rates in Idaho have been higher than the U.S. and in 2007 and

2008 the rates in Idaho (91.9 and 83.5, respectively) were higher than the U.S. preliminary 2007

rate of 49.1. Alternatively, the rate for infant deaths due to Other symptoms, signs and abnormal

clinical and laboratory findings, not elsewhere classified, such as SIDS, have been higher in the

U.S. (47.3 for 2007 preliminary) than in Idaho (none in 2007 and 4.0 per 100,000 in 2008).

In 2008, the leading single cause of neonatal mortality was Congenital malformations,

deformations and chromosomal abnormalities (25 deaths); with a neonatal death rate of 99.4 per

100,000 live births. SIDS was the leading single cause of death in the postneonatal period at 75.5

postneonatal deaths per 100,000 live births, accounting for 40.4 percent of all postneonatal deaths.



Stillbirths and Perinatal Deaths

Idaho reportable stillbirth deliveries are defined as a spontaneous fetal death of twenty (20)

completed weeks gestation or more, based on a clinical estimate of gestation, or a weight of three

hundred fifty (350) grams (12.35 ounces) or more. In 2008 there were 128 stillbirths to Idaho

residents; the ratio of stillbirths to live births was 5.1 stillbirths per 1,000 live births. In the past ten

years, the stillbirth ratio ranged from a low of 4.5 per 1,000 live births in 2006 to a high of 6.3 per

1,000 live births in 1998 and 2001.



Perinatal death is defined as the number of stillbirths plus the number of deaths to infants

fewer than seven (7) days of age per 1,000 live births. The perinatal ratio is often used to compare

U.S. data with other countries because of differences in the definitions of live birth, stillbirth and

infant death among countries. In 2008, there were 217 perinatal deaths to Idaho residents. The

Idaho perinatal death ratio decreased 2.2 percent from 8.8 per 1,000 live births in 2007 to 8.6 per

1,000 live births in 2008.



Maternal Deaths

Maternal causes of death include pregnancies with an abortive or live outcome and include

direct and indirect obstetric causes and complications. The World Health Organization defines

maternal death as “the death of a woman while pregnant or within 42 days of termination of

pregnancy, irrespective of the duration and the site of the pregnancy, from any cause related to or

aggravated by the pregnancy or its management, but not from accidental or incidental causes.”1



In 2003, the Idaho death certificate was revised to include a separate question regarding

pregnancy status of females aged 10 to 54. Data are reported if female was pregnant at time of

death, within 42 days of death, or 43 days to one year prior to death. Data from the response to

this question are used in conjunction with cause-of-death information reported in Part I and Part II

on the death certificate to determine the underlying cause of death. This separate pregnancy status

question on the death certificate resulted in the identification of more maternal deaths in states with

the revised death certificate1. Idaho maternal death data in 2003 and later are not comparable with

Idaho data prior to 2003, the U.S., or states not using the revised death certificate.



In 2008, there were 4 maternal deaths to Idaho residents (death rate of 15.9 deaths per

100,000 live births). The ICD-10 codes used for maternal deaths are A34, O00-O95, and O98-

O991. In addition to the 4 maternal deaths during pregnancy or within 42 days of termination of

pregnancy, there were 2 deaths due to pregnancy, childbirth, and the puerperium which occurred

43 days to one year after delivery or termination of pregnancy. The ICD-10 codes used for these

deaths are O96-O97 and these deaths are not included in the count or the rate for maternal deaths.



In the U.S. in 2006, there were 569 maternal deaths (death rate of 13.3 deaths per 100,000

live births) and 191 deaths to women due to maternal causes which occurred 43 days to one year

after delivery or termination of pregnancy. As of 2006, 32 states and the District of Columbia have

a separate question related to pregnancy status of female decedents at the time of their death1.
