CONSULAR OUTREACH IN OHIO
The Philippine Consulate General in Chicago is pleased to announce that it will render
consular services at Columbus, Ohio on 29 August 2009 at the La Herba Buena Filipino Store,
6727 Dublin Village Drive, Dublin Village Shopping Center, Dublin, Ohio 43017 , from 9:00 a.m.
to 5:00 p.m.
The following services will be provided by the Consular Team: (1) Passport; (2) Visa; (3)
Notarials/Authentication; (4) Dual Citizenship; (5) Filing of Reports of Birth/Marriage; and (5)
Registration of Overseas Absentee Voters (for the 2010 Presidential/ Senatorial elections).
For PASSPORT AND DUAL CITIZENSHIP APPLICANTS, please mail/fax your application
forms and supporting documents to the Consulate before the scheduled date of the consular
outreach for pre-processing of your applications. You are, however, still required to appear at
the venue to complete the following requirements: 1) personal appearance; 2) signature and
thumbmarks, 4) presentation of original documents/requirements; and 3) payment of fees.
For OVERSEAS ABSENTEE VOTING REGISTRATION, the requirements are as follow:
1) Personal appearance/submit self to biometrics capture for photo, thumbprints and
signature; 2) copy of valid Philippine Passport; 3) For seafarers, a copy of Seaman’s Book; 4) for
Dual Citizens, a copy of Oath of Allegiance/Order of Approval.
