Eligibility
2 X basic annual earnings to a maximum of $600,000
Life/Accidental Death & Guarantee Issue: $600,000
Dismemberment Insurance Coverage will reduce on age 70 to 65% of the original amount. On age 75, the
benefit coverage will reduce further to 50% of the original amount.
If you become terminally ill and are not expected to live more than twelve
months, you may request 75% of your death benefit to a maximum of $500,000.
Accelerated Death Benefit A doctor must certify your condition in order to qualify for this benefit.
Benefits over $10,000 are paid through the UNUM Security Account. This interest
bearing account will be established in the beneficiary's name. He or she can
Security Account then write a check for the full amount or write checks on the account as needed.
If you retire, reduce your hours or leave your employer, you can take this
coverage with you according to the terms outlined in the contract. However, if
Life Insurance Conversion
you have a medical condition, which has a material effect on life expectancy,
and Portability you will be ineligible to port your coverage.
Survivor Support is a personalized financial counseling service that provides
survivors and terminally ill employees with support relative to their immediate
and future financial needs. Supplied with information provided in advance by
you, the Ayco counselor begins the relationship with the survivor or terminally ill
employee by meeting, one on one, and reviewing their financial situation and
concerns, analyzing alternatives, making recommendations and developing
strategies to resolve issues.
