For clinical          The Department of Medicine has implemented a work relative value unit (wRVU) based
faculty               compensation plan that determines the HSF component of a faculty member’s salary and
                      rewards clinical productivity. Although your HSF salary for the first year will be guaranteed
                      to be at least $______, it could be higher based on your clinical productivity. Incentive
                      payments are determined and paid on a quarterly basis. After the initial year, your HSF
                      salary will be determined by your clinical productivity.

Responsibilities

                   You will be responsible for (Spell out the division’s quantitative and qualitative

                   The Department of Medicine expects all faculty to be productive, effective, and
                   excellent in their areas of responsibility as outlined above. This expectation includes
                   alignment of your activities with, and generation of, support for these activities and
                   relates to UAB salary in support of academic efforts, HSF salary in support of clinical
                   efforts, and VA salary in support of VA efforts. The UAB Department of Medicine has
                   a stated expectation that each faculty member should cover a minimum of 75% of
                   his/her academic salary, and 100% of his/her clinical salary and costs of research
                   programs. The UAB Division of ________ will commit to coverage of ~##% of your
                   salary (~$##,### per year), for the first three years of your faculty appointment, with the
                   remainder of your salary coming from clinical and educational activities as outlined above.
                   This protected time will allow you to develop your educational portfolio, research
                   capabilities, or other professional activities. The departmental standard for outstanding
                   performance by established faculty is attainment of support approaching full coverage of
                   total salary.

Requirements       This appointment is contingent upon your obtaining and maintaining a valid license to

                   offer, it is your responsibility to seek licensure and privileges in an expeditious manner,
                   since your appointment will not be final without them.

                   your UAB/HSF appointments, in accordance with Section 2.6 of the Faculty Handbook
                   and the HSF employment agreement, respectively. In the event your VA employment
                   is terminated or reduced for any reason without the agreement and written consent of
                   the UAB Division Director and the Department of Medicine Chair, your UAB/HSF
                   salary will not be increased to make up for the loss of the VA salary.

                   The School of Medicine requires that the enclosed Faculty Practice Form be completed,
                   notarized, and returned directly to the Dean’s Office as noted on the form. Should you
                   accept our offer, it is your responsibility to seek licensure and privileges in an
                   expeditious manner, since your appointment will not be final without them.

                   institution we are also required to have verification of credentials from an outside
                   agency, AMA certification, or ECFMG certification.

                   All faculty appointments must receive final approval from the Dean, Provost, and
                   President. We anticipate no difficulty with these steps. If this offer is acceptable to

Please note that the University of Alabama at Birmingham conducts a review of the
Office of Inspector General (OIG) and the General Services Administration (GSA)
exclusion lists to establish eligibility for each faculty member to receive federal
funding. These databases are maintained by the Federal Government to identify
individuals who are excluded from participating in payments by Federal programs or
contracts. Your employment may be contingent upon a satisfactory report. Every
effort will be made to conduct this review prior to the beginning of employment. By
signing this letter of offer, you are giving UAB permission to conduct this review.
