Theatre Camp REGISTRATION and cancellation agreement FOR 2008!

PLEASE READ, SIGN, AND RETURN THIS FORM TO THE QUICK CENTER.



The registration of eligible candidates for the 2008 summer theatre camp presented by the Quick Center for the Arts at

Fairfield University will be determined through a lottery-style registration to ensure the registration process is completely fair

and equally accessible for every potential participant. The registration lottery will take place at the Quick Center for the Arts at

5:00p.m. Monday, March 10, 2008. Initial enrollment in the Missoula Children’s Theatre Workshops will be based upon the

results of the lottery and will NOT be dependent upon the order in which the registrations are received. Attendance is NOT

required at the lottery drawing on Monday, March 10, 2008.

To be eligible for the lottery, the Quick Center Box Office must receive your completed Registration Form, total

tuition payment(s) for all applicants, and a signed copy of this form before 2:00p.m. Monday, March 10, 2008.

Registration Forms received after 2:00p.m. on Monday, March 10, will be added to the theatre camp’s waiting list after

the lottery process.

All Registration Forms, along with the required tuition payment in full for each applicant (credit card or checks

only), should be mailed to: Quick Center for the Arts, Fairfield University, 1073 North Benson Road, Fairfield, CT

06824-5195, Attn: Box Office MCT. Registration Forms may be delivered in person, along with the required tuition payment,

to an official Box Office representative at the Quick Center during regular Box Office hours. (Not responsible for Registration

Forms given to any other Quick Center staff prior to the opening or after the closing of the Box Office. Not responsible for

applications not received in time through the mail). A signed copy of the Registration Agreement and Cancellation Policy must

be submitted to the Quick Center Box Office with your Registration Form. Registration Forms will NOT be accepted by

telephone, fax, or e-mail, prior to the registration lottery on Monday, March 10, 2008.

ONE REGISTRATION FORM ALLOWED PER FAMILY. Each child’s participation is limited to one session,

depending upon availability. Please be certain that your children’s session preferences are clearly indicated on the Registration

Form. Number each child’s order of preference for sessions (#1 for first choice, #2 for second, etc.) and cross out any session in

which the child will not be able to participate. If your first choice has already been filled, the applicant will be placed in another

chosen session. If all preferred sessions are filled, the applicant will be added to the waiting lists for all selected sessions.

Upon receipt, each Registration Form will be assigned a “Lottery Number” which will be the applicant’s lottery number

for the registration drawing. Each family will be assigned one “Lottery Number” only for the drawing.

When all available openings are filled, the remaining registrations will be drawn to determine placement on

the waiting list. Tuition payments will be processed upon confirmation of registration in a selected session during the

week following the enrollment lottery. Participants will be notified of their placement in the program by

e-mail. A first time participant’s birth certificate will be required upon official confirmation of registration.

Registration Agreement & Cancellation Policy

Missoula Children’s Theatre Camp 2008



Each registered participant will be at least six (6) years old by July 1, 2008 and entering at least the first grade in the fall.

A copy of each participant’s birth certificate must be provided to the Quick Center if one is not on record. The required birth certificate

should be that of the original registered camper, the same camper who will be participating in the registered session.

A $25.00 fee will be charged in the event of any cancellation, transfer, or the approved substitution of any participant.

Payments may be transferred to another session on behalf of the camper, depending upon availability, only with the prior approval of the

Quick Center at least one full week before the registered session is scheduled to begin.

NO CANCELLATIONS/TRANSFERS/SUBSTITUTIONS of any participant will be permitted without the prior approval of the Quick

Center. Approval must be secured at least one full week before the registered session and will be dependent upon availability and the status

of the official Missoula Children’s Theatre Camp Waiting List maintained by the Quick Center Box Office.

In the event of a cancellation at least one week prior to the camp session, refund of the remaining tuition (minus a $25 cancellation fee per

camper) will be subject to the securing of a replacement camper for the session by the Quick Center.

Absolutely NO cancellations, transfers, substitutions, or refunds are possible within one week of the session or once the session has begun.

I agree to notify the Program Coordinator on the first day of the session of any allergies or medical conditions of any camper.
