1. PURPOSE OF THE JOB
As part of the management team at the extra care service to provide
leadership, the provision of personal care, basic health care and practical
support as determined by assessed needs of the tenants living in the scheme,
/ service users living within the local community to enable individuals to live as
independently as possible in their own home.
2. PRINCIPAL DUTIES AND RESPONSIBILITIES
a) Departmental
1. To participate in the formulation of detailed Team objectives and
policies.
2. To ensure the effective and efficient implementation of Company
policies and the achievement of the Company's objectives, including
financial ones
3. To ensure that holistic care and support is provided to meet all tenants’
/ service users’ physical and psychological needs and to promote the
continuing spiritual, family and social life of the tenants / service users,
helping them to maintain social interaction with family, friends and the
community in line with their wishes and preferences.
b) Functional
a) To assist the Extra Care Manager in the day to day running of the
service via an electronic booking system.
b) To assist the Extra Care Manager to manage and monitor the
effectiveness of the key worker system within the extra care scheme,
including the preparation, implementation and review of individual care
plans and risk assessments, and attending reviews.

c) To assist the Extra Care Manager to plan work and work rosters, and
manage each shift deploying staff effectively and efficiently, providing
the required cover and taking lead responsibility (in the Manager’s
absence) in this role. This will be done via an electronic booking
system.
d) Develop appropriate and professional relationships with tenants /
service users and their carers, which show commitment to valuing
users as individuals, including establishing and maintaining effective
communication
e) Support and motivate tenants / service users to maintain personal
independence and social interaction consistent with their own abilities
and preferences.
f) To supervise and provide the highest practicable standard of care and
support for tenants / service users and act as a role model for support
worker who will undertake the role of Key Worker for a specified
number of tenants / service users.
g) To be responsible for the supervision of the scheme in the absence of
the Extra Care Manager. This will include deploying staff resources
effectively via an electronic booking system and being the first point of
contact for dealing with all emergency situations. Also dealing with
enquiries about potential new service users, organising visits and
assessments.
h) To understand and be responsible for safe working practices including
carrying out and implementing risk assessments bringing concerns
quickly to the Extra Care Manager’s attention if unable to resolve them
at the time.
i) To promote and encourage effective teamwork
j) To work flexible hours to include evenings and weekend working and
participate in a rota. To be able to respond to emergency situations
and provide cover for other staff where necessary (this may be in
another Extra Care Scheme if needed to respond to an emergency
situation)
k) To supervise Support Workers and Night Support Workers in their
responsibility for individual tenants / service users.
l) To carry out pre-admission assessment visits to prospective tenants /
service users where appropriate and necessary, and to prepare and
present relevant assessment information in writing.
m) To order repeat prescriptions for tenants / service users in line with
their wishes and as prescribed by their medical practitioner. To
administer and/or offer medication strictly in accordance with the
Company’s medication policy.

n) To contribute to the control of financial systems, in line with audit
requirements as set out in Company policies and procedures.
o) Under the direction of the Extra Care Manager to assist in budgetary
controls
p) In accordance with the individual tenant’s / service user’s care/service
plan to supervise and participate in the provision of personal care and
support to individuals. This could include, for example, help with:
Getting up/putting to bed
Personal hygiene
Dressing/undressing
Use of toilet/commode
Catheter care including stoma and sheath care
Assisting with prostheses (false legs/eyes etc)
Application of simple dressings
Using sterile techniques
Foot and hand care
Care during illness and terminal illness
Health promotion
Person centred support for those with dementia, mental health
problems and for those with learning difficulties
Assisting tenants / service users to eat and drink which includes
preparation of simple snacks and beverages and service of food (at
the place where tenants choose to eat)
q) Assist with identified activities as set out within social and health care
rehabilitation programmes, enabling tenants / service users to
maximise their independence; whether this be within the Extra Care
Scheme itself or the local community.
r) To maintain high standards of confidentiality at all times
s) Contribute to the protection of individuals from abuse through the
adherence to the Safeguarding Adults Policy and at all times relating
any issues or concerns to a line manager and to undertake lead
responsibility in this role out of hours
t) To promote, monitor and maintain health, safety and security in the
workplace in accordance with Company procedures
u) To liaise with emergency and support services as necessary including
ambulance, fire brigade, property services and police and take a lead
in this role, out of hours
v) Participate in and lead as directed by the Extra Care Manager regular
service and quality reviews
w) Keep up to date written records about individual tenants / service users
and maintain confidentiality of information in accordance with Company
Policy and Data Protection legislation

x) Complete any administrative procedures and related forms (for
example daily record sheets, hand-over sheets, timesheets, and
accident and incident forms) and use appropriate information as
required
y) Assist in the training and take a lead role in the induction and
professional development of new staff and the promotion of safe
working practices, contributing to the implementation of Health &
Safety at work
z) Contribute and participate in individual reviews, reports and care plans
as required. Will on occasion be required to chair review meetings.
3. QUALIFICATIONS, KNOWLEDGE AND EXPERIENCE
a) Possession of Level 3 Diploma in Health and Social Care would be an
advantage or ability and commitment to obtain same within the first two
years of appointment (whether permanent or casual) is essential.
b) Possession of an NVQ (Care/ Promoting Independence) Level 3 or a
willingness to obtain the Level 3 Diploma in Health and Social Care is
essential
c) Personal experience in a similar care setting, including life experience
would be essential
d) Demonstrate appropriate level of computer-based skills (e.g. CLAIT) in
order to use the electronic booking system appropriately.
e) Be trained in First Aid to an appropriate level
4. DIMENSIONS
Responsibility for supervising Support Workers / Night Support Workers
Responsibility for managing each shift, deploying staff and providing the
required cover - taking lead responsibility (in the absence of the Extra Care
Manager) in this role
5. PHYSICAL EFFORT
The post holder will more than likely have to help tenants / service users out
of bed, help with dressing and undressing which will mean bending and lifting.
The post holder will be responsible for using bathing and lifting equipment.
The post holder will be prepared to potentially support service users living
within the community local to the Extra Care Scheme.
6. Working Environment

The post holder will have to provide personal care and emotional support for
tenants / service users which could mean contact with a combination of noise
and odour.
7. General
a) This job description contains the principle duties of this post. The post
holder maybe required to carry out other duties that are commensurate
with this grade.
b) The post-holder will be required to respond to urgent situations and
provide cover for other members of the team. This may include the post
holder agreeing to be available for cover during the night on an occasional
basis
