DR. BRIAN IRWIN
Don’t get cold feet
Most patients will experience a sensation of cold feet (or hands)
that seems to come on for no particular reason during their life-
times. While it may not bother the patient, very frequently the
partner they sleep with will bring it up. I’ve had a number of
patients present with a chief complaint of, “My wife says my feet
are freezing!”
More likely, however, the “cold feet” do bother the patient,
because they’re uncomfortable. Cold feet are also often concerning
to patients, as they may make a person worry about the adequacy
of their circulation. While poor circulation is one possible cause of
cold feet or hands, there are many others.
The most important thing to evaluate when you sense unreason-
ably cold feet is their actual temperature. If the feet are actually
cold when pressed against another part of your body (the inside
of your knee, for example) the possible causes are very different
than if they are normal in temperature but simply “feel cold.” In
the latter situation, the condition may actually be related to the
nerves that supply the foot. These nerves may be impinged upon
as they exit the spine; they may be damaged due
to a number of diseases like multiple sclerosis or
diabetes, or they may be affected by a long list
of inflammatory or infectious conditions. Under
these circumstances, it’s best to see the immediate
attention of a health-care provider.
Much more commonly, the feet actually are cold.
Again there’s a long list of possible causes for this,
many of which are quite rare. Poor circulation can Hospital volunteer Maryann Hartigan, center, presents a donation for the First Baby of 2007 from Suncatcher Gift

of poor circulation is peripheral vascular disease. This condition,
which is really a spectrum from arterial blockage to venous insuf-
ficiency can be caused by diabetes, obesity, high cholesterol, among
other diseases. Hyper-reactive arterioles (the microscopic vessels

that feed capillaries) may spasm, leading to an isolated cold toe or
cold entire foot. This can be due to a condition known as Raynaud’s
disease, damage from previous frostbite, trench foot or as a reac-
tion to certain medications, just to name a few causes. Abnormal
proteins in the bloodstream, known as cryoglobulins, exist in some
people and precipitate out of solution if the blood temperature
drops, leading to “clumping” within the arteries and subsequent CONWAY — The Memorial Hospital in According to Sharon Malenfant, the
