2011 VENDOR BOOTH APPLICATION



The Oklahoma Czechs, Inc. and the Yukon Chamber of Commerce cordially invite you to

participate in the Oklahoma Czech Festival on Saturday, October 1, 2011 from 8:00 a.m. to 5:00

p.m. Included with this application is a layout of the booth spaces with the numbers indicated.

Booth spaces are strictly assigned by when your completed application is received. The booth

application is now online at www.yukoncc.com. After the online application is received, you will

receive an invoice for payment. All payments must be received by August 31, 2011. Also, booth

availability and assignments will be posted online. Once you have been assigned a booth, you will

receive acknowledgement by e-mail. If you do not have an e-mail address, please send a self-

addressed stamped envelope with your completed application.

Please note: Craft booths and food vendors must contact Don Richart, OK Czechs (405) 722-1004

for an application. This application is NOT for food vendors.



DATE OF FESTIVAL: Saturday, October 1, 2011



LOCATION: 4th and Main Street – Yukon, Oklahoma



FESTIVAL HOURS: 8:00 a.m. to 5:00 p.m.



BOOTH SPACES: Booth spaces are 10’x10’



BOOTH RENTAL FEES: $ 65.00 Yukon Chamber of Commerce Members

$ 85.00 for non-members



PAYMENT METHODS: Cash, Check, Visa and Master Card

Chamber Members only may be invoiced.



APPLICATION DEADLINE: August 31, 2011



Booth assignments will be based on the date the applications are received. Chamber Members can

request to be invoiced for their booth. Non-members must send payment in full with their completed

application. All booths must be paid in full by August 31, 2011. Late applications will be accepted

after the deadline ONLY when accompanied by full payment.



VENDOR SET-UP: Friday, September 30, 2011 after 5:00 p.m. or

Saturday, October 1, 2011 from 6:00 a.m. to 7:45 a.m.

(Streets are closed at 8:00 a.m.)



Bring your chairs, tables, display panels, the booth diagram, and other associated items you may

need for your display. After unloading, move your vehicle to allow others to unload in their assigned

booth. Due to traffic congestion, all vendors should have their booths set up by 7:55 a.m. on

Saturday. ELECTRICITY IS NOT PROVIDED.



CANCELLATIONS/REFUNDS: Allowed until August 31, 2011.



INCLEMENT WEATHER: The festival is held "rain or shine," so be prepared. No refunds

will be issued due to inclement weather.



SALES TAX: Vendors are responsible for collecting and paying sales tax to

the Oklahoma Tax Commission.

The Oklahoma Czechs, Inc., the Yukon Chamber of Commerce and the City of Yukon will

not be responsible for any loss or damage suffered by vendor, vendor's guests, invitees, employees,

agents or subcontractors. The undersigned vendor agrees to protect, defend, indemnify and hold

harmless the Oklahoma Czechs, Inc., the Yukon Chamber of Commerce, and the City of Yukon and

their members from and against any and all claims, demands, damages, losses, liabilities, causes of

action, suits and expenses, including attorney's fees, court costs and other expenses of litigation of

every kind and character, without limit and without regard to the cause or causes thereof or the

negligence of any party or parties, arising in connection herewith in favor of vendor, vendors' guests,

invitees, employees, agents or subcontractors or their employees, on account of bodily injury, death,

or damage to property.



By signing below and submitting this Vendor Booth Application, I acknowledge that I have
