Directions: Observation and assessment of children allows a teacher to evaluate a child’s progress, create goals and planned learning
opportunities for that child, while sharing the program perspective and gaining parent ideas about the child’s experience is key to forming
relationships that benefit the child. This optional tool allows you to track the observation, assessment and parent conference history of the
children in your classroom. Columns correspond to the Performance Standards requirements for each STAR level. For each level, you must
complete the columns for that level and all lower levels. If your facility meets some or all of the higher observation/assessment levels, please
complete these columns as well to insure you are consistently meeting the needs of the children and parents.
