Description: Child & Family Support Services is hiring immediately for an Integrated Health Support
Specialist position in the Greater Flagstaff area. As part of an effort to develop integrated health care
and community-based support for children within the behavioral health system, Child & Family Support
Services has been granted the unique opportunity to develop a program in the Flagstaff area called Rock-
It. Rock It will incorporate Physical Activity, Psycho –Education, Family Support, and Community
Partnerships. The Integrated Health Support Specialist position will be flexible to accommodate a
program that will be accessible to children and their families around their availability. This will include
evening and weekend work.

Duties for this position will include:
Establishing relationships and partnering with young people and families
Designing and implementing creative activities for Children and Families
Measuring progress and outcomes of services provided to young people and families
Communicating effectively with families, community partners, Child and Family Team members
(stakeholders, therapists, case managers, etc), agency management and support employees
Providing direct support to young people and families
Supporting and adhering to company policies and procedures
Modeling company values
The position will require a variety of skills such as active listening, communicating using different formats
(face-to-face, email, telephone, etc), reading comprehension, verbal and written expression; critical
thinking, time management, social perceptiveness, and problem solving. Desired work styles for this
position include dependability, integrity, attention to detail, cooperation, concern for others, initiative,
leadership, and adaptability. It also requires an ability to manage conflict with a high degree of
professionalism.
Requirements: All applicants must be 21 years or older and have a high school diploma or equivalency.
Bachelors’ Degree required. Applicants must demonstrate an ability to relate to children and adults, to be
comfortable working in a strength-based and family-centered way, and be able to work effectively within a
team environment. Flexibility in scheduling, and willingness to work within the homes and communities of
the families we work with is required. Candidates are required to submit a negative substance abuse test,
be able to obtain fingerprint and criminal history background clearance. CPR, First Aid, and Therapeutic
Crisis Intervention are required trainings that are provided by the agency. Proof of reliable transportation
is required, including proof of insurance and driving record.
Salary is DOE.
