Registrations for Code 1 Shipments

Online Registration Form, you must accept terms to continue.
Fill out form, click submit. If you have any problems, please call
Registration Clerk, 1-800-365-5463.
The registration form is broken into four sections:
#1-Shipper Information
#2- Origin Information
#3- Destination Information
#4- Shipment Descriptions
The following is required information to process the order completely:
Section #1- Shipper Information
a) Shipper’s name & Rank
b) Carrier’s name & SCAC
c) GBL #
Section #2- Origin Information
Code 1 Operations
a) Origin address & Phone
b) County code (Contact registrations)*
c) O/A code and name (Contact Agency Dev)*
d) Origin Base
*Your agency should have a listing of county codes for your county. The O/A
code is your agent #, which is issued by Agency Development. If you do not
know your agent #, please call your Agency Representative at 1-800-365-5463
and we will issue one. NOTE: United Agents’, use their agent number for O/A
code*
Section #3- Destination Information
a) Complete destination address & Phone
b) Destination base (as indicated on booking order)*
* Military Operations will assign the destination agent.*
Section #4- Shipment Information
a) pack, load, RDD dates
b) weight of the shipment
c) discount rate
d) open, request to haul or self haul *
*If you would like to self haul the shipment, select “self haul”, along with van line authority.
“Request to haul” pertains to United and Mayflower agents only. “Open haul”, we will assign a
truck to haul, and communicate the van assignment via phone call or email.*
REMINDERS when registering a shipment:
♦ Shipments containing firearms should be specified on 1299 and/or GBL. These are required
fields on registration form.
♦ Please list any items on the shipment that may need special attention in the “special
instructions” box with a complete description.
♦ At the bottom of the form is the agent contact information. The move manager or
coordinator sending in the registration should fill this area out completely.
Code 1 Operations
♦ Make sure you press the “submit” button on the bottom of the page. You should receive a
confirmation sent back to the e-mail address you submitted in the agent contact information
area.
The confirmation will include the order number and destination agent assigned to the
shipment. If you do not receive a confirmation email or fax within 48 hours, please contact
Registrations Clerk, 1-800-365-5463.
♦ Please send in writing, any adjustments made to the shipment (weight changes, date changes,
etc.)*
♦ Please fax or email PRE-MOVE SURVEY’S . *
*These changes should ALL be in writing, faxed or emailed to: sovermeyer@suddath.com
,rpierce@suddath.com, or djames@suddath.com. *
♦ Failure to fully comply with all rules, regulations, procedures, and duly filed and current
tariffs of Carrier/Forwarder(i.e. pre-move survey), may result in a reduction of agent's
booking commission.
♦ Any cost associated with an APU (Agent Pickup) on an overflow, will be charged back to the
origin agent if not accompanied by a timely submitted pre-move survey and/or written
notice of the overflow to the carrier within 2 days.
