NOMINATION FORM INSTRUCTIONS Community Foundation of Acadiana’s (CFA) mission is to improve the quality of life in Acadiana by increasing giving, connecting donors to the causes they care about and providing leadership on community needs and opportunities. CFA will recognize one group or individual at its 2011 Leaders in Philanthropy Awards Luncheon on Thursday, November 10, 2011 from 11:30am-1:30pm from the parishes of Acadia, Evangeline, Iberia, Lafayette, St. Landry, St. Martin, St. Mary, and Vermilion. CFA is accepting nominations for an individual or group* who, through their efforts in philanthropy, has created an impact in their community. Nominations will be accepted from CFA Fund owners, Friends of the Community Members, Board and Affiliate Board of Directors, Committee Members and Sponsors. If you are not affiliated with CFA in one of these ways, you may submit an application through someone you know that is affiliated with CFA. Or, you can become a Friend of the Community Member today by visiting www.cfacadiana.org/about.
