									              *North Fork Music Boosters Craft Show*
   SATURDAY ONLY - December 10th, 2011, 9am to 3pm
   Newton Elementary, 6645 Mt. Vernon Rd., Newark, OH

General Information:
Application Deadline: November 15th, 2011 - All applications must be
postmarked. We do not accept electronic/e-mail submissions. Please note that
submitting an application does not guarantee acceptance. If we receive more
artist/crafter requests than we can accommodate in the available space we will
make selections based upon application postmark date.

Artist/Crafter Notification Deadline: November 20th, 2011 - If openings occur, we
will draw from our wait list to fill those openings.

Booth Fee: $40.00 per booth (size 10'x10') $30.00 per booth (8'x6')

Electric Fee: $10.00 per application (must be on an outside wall to have electric)

Refunds: All cancellations must be in writing. Cancellation deadlines are as
follows: 100% refund before November 15th, 50% refund before November 20th,
no refunds given for cancellations received after November 20th.

Setup Dates/Times & Registration: Friday, December 9th, 6 - 8pm and
Saturday, 7 - 9am.


    All exhibitors are expected to present their artwork/crafts in an attractive manner with
     covered tables and inviting displays. Displays must fit the booth size.
    If approved, all food items must be wrapped/packaged for consumption away from the
     craft show (i.e. candy, cookies, etc.)
    Crafters/artisans are expected to leave their space neat and clean at the conclusion of the
     show. Spaces are not to be dismantled until after the 3pm show closing time.
     Crafters/artisans failing these terms will not be permitted a space at future shows.

Description of craft items to be sold:

I have read the attached guidelines and agree to comply with these terms. I
understand that by not complying with these terms I may be excluded from future
craft shows. I furthermore understand that I am responsible for maintaining my
space for the duration of the show and that neither the North Fork Music Boosters
nor the North Fork Local School District will be held Responsible for any injury,
loss and/or damage which may occur for any reason.
